SUBJECT: NRC INSPECTION REPORT 50-458/98-04 AND NOTICE OF VIOLATION
Dear Mr. McGaha:
From January 11 through February 21,1998, an inspection was conducted at your River Bend
Station reactor facility. The enclosed report presents the scope and results of that inspection.
During the 6-week period covered by this inspection, your conduct of activities at the River Bend
facility was generally characterized by safety-conscious operations, sound engineering and
maintenance practices, and good radiological controis. However, one violation was identified
during this inspection period for the failure to follow design drawings which specified that
petcock valves on containment isolation dampers were required to be at least one full turn open

when the dampers were operated in the safety-related mode. As a result, Damper HER-AV-165
was found to be inoperable during surveillance testing. Although the violation appears to meet
our criteria for enforcement discretion, we have decided to cite the violation because of the self-
disclosing nature of the issue, the consequences associated with the inoperability of the
containment isolation damper, and the weaknesses in your controls that reduced the ability to
identify this problem before it resulted in rendering the damper inoperable.
The violation is cited in the enclosed Notice of Violation (Notice) and the circumstances
surrounding the violation are described in detailin the enclosed report. Please note that you are
required to respond to this letter and should follow the instructions specified in the enclosed
,
Notice when preparing your response. The NRC will use your response, in part, to determine

whether further enforcement action is necessary to ensure compliance with regulatory
requirements.
In addition, we would like to meet with you to discuss the accessibility of your auxiliary building
following a design basis event. We have dstermined that it is difficult to open the doors leading

to the auxiliary building when even one train of the standby gas treatment system is running.
This poses a potentially significant challenge to operators while attempting to enter the building
to implement emergency operating procedures and abnormal operating procedures. While we
recognize that you have taken measures to ensure that operators can attain access through two

pathways, we believe that these actions are only adequate for the short term. Based on
discussions between Mr. Elmo Collins of my staff and Mr. David Lorfing of your staff, the
meeting is scheduled in our regional office in Arlington, Texas, on March 24,1998. For this
meeting, please be prepared to discuss: (1) the configurations of the standby gas treatment
system that you believe could cause accessibility problems; (2) the emergency actions

by accessibility challenges and those actions that are time sensitive; (3) whether the current
standby gas treatment system performance is consistent with the design bases; (4) what you
believe to be the cause of the accessibility problem; and (5) your planned actions to correct the
problem.

enclosure (s), and yow response will be placed in the NRC Public Document Room (PDR).
