l SUBJECT: NRC INSPECTION REPORT 50-10/98002(DNMS) AND NOTICE OF VIOLATION

Dear Mr. Kingsley:

On February 26,1998, the NRC completed an inspection at your Dresden Unit 1 Station which

examined decommissioning activities. The enclosed report presents the results of that

,

inspection.

!

During the (11-week) period covered by this inspection, activities in the areas of facility

management and control, decommissioning support, and radiological safety were examined.

Overall, the performance of reactor decommissioning activities was satisfactory. However, the

Radiation Protection program exhibited a number of weaknesses, most of which were self

identified and addressed. The Radiation Protection program had declined through most of 1997  ;

until controls had become informal, personnel were not well informed or disciplined, and i

i management involvement and assurance of quality were lacking. Subsequently, improvements

were evident.

Two violations of NRC requirements were identified during the inspection. The first violation l

involved a failure to meet Technical Specifications regarding the locking of a High Radiation

Area. The second violation involved the identification of multiple failures to follow procedures,

which was also a violation of Technical Specifications. These violations are of concem because

they indicate a low expectation level existed concoming radiological practices on Unit 1, which

resulted in decreased personnel diligence and attentiveness to their radiological work practices.  ;

We acknowledge the thoroughness of their investigations of these issues as well as your

aggressiveness in addressing the issues and in raising program expectations. Nevertheless, we

believe that additional improvements are still warranted. In addition, we believe you need to

improve your ability to identify weaknesses while they are still minor issues. This is a concem

' because many of the recently addressed program weaknesses were identified only after an 1

extensive investigation that was precipitated by an intake of radiological contamination. Had this / )

intake not occurred, the investigation in all likelihood would not have occurred, leaving these  !

issues unidentified.

The violations are cited in the enclosed Notice of Violation, and the circumstances surrounding

the violations are described in detail in the enclosed report. Please note that you are not

required to respond to this letter in response to the enclosed Notice. Based on our inspection

l results, the corrective actions you have already taken were considered to be sufficiently

!: comprehensive to address the root causes for the violations and to prevent recurrence. .
