Modification No.1 to Task Order No.11 Under Contract No. NRC-26-94-263 This confirms the June 1,1998, verbal authorization provided to you under the subject task order modification with a temporary ceiling of $2,219.00 to:
(1) add an additional instructor to the June 1-5,1998, Root Cause/ Incident investigation Workshop (Region l} provided for under the original Task Order No.11(this is an increase of
$2,219.00 in travel costs for this course);
(2) add 5 students to the June 8-12,1998, Root Cause/ Incident Investigation Workshop (Headquarters) provided for under the original Task Order No.11. This brings the total number of students for this class to 20 students. 20 students requires 2 instructors therefore, this modification is a'so providing for an additional instructor as well (this is an increase of $5,400.00 to this course and $2,186.00 in travel costs for a total increase of $7,586.00), and :
(3) provide for a July 20-24,1998, Root Cause/ Incident investigation Workshop (Headquarters) for 24 people which will therefore require 2 instructors ($30,140.00 for this course and I
$5,768.00 in travel costs for a total of $35,908.00).
The above total additional costs of $45,713.00 hereby increases the ceiling and obligated amounts of this task order from $95,501.00 to $141,214.00. Accordingly, paragraphs no. 2 and 3 of Task Order No.11 dated February 11,1998, are hereby deleted in their entirety and the following substituted in lieu thereof:
" Task Order No.11 shall be in effect from March 9,1998, through September 30,1998, with a ceiling of $141,214.00 which is inclusive of the $2,219.00 authorized on June 1, 1998. The amount of $121.304.00 represents the total firm-fixed-price for the courses as stated in the enclosed Statement of Work. The amount of $19,910.00 represents the total estimated reimbursable costs for travel associated with this task order."
The obligated amount of this modification is $45,713.00.

l TASK ORDER NO.11 MODIFICATION 2 UNDER CONTRACT NO. NRC-26-94-263 Root Cause / Incident investigation Workshop FY98 1.
PURPOSE The Nuclear Regulatory Commission (NRC) licenses and inspects reactor and non-reactor facilities to ensure compliance with applicable codes and standards during all phases of construction, testing, and operation in order to identify conditions which may adversely affect the health and safety of the public. In order to ensure that the Investigation of events is carried out in a structured, coordinated and integrated manner, I
the NRC's Office for Analysis and Evaluation of Operational Data provides formal training l
in incident investigation.
The primary objective of Task Order 11 rnodification 2 under Contract NRC-26-94-263 is to provide training necessary to ensure that operational events are investigated in a systematic and technically sound manner to gather information pedaining to the root 1
causes of the events, including any NRC contributions or lapses, and to provide l
appropriate feedback regarding the lessons of experience to the NRC, industry, and to the public.

SCOPE OF WORK W
Add a new Root Causelincident Investigation course for Headquarters July 20-24, 1998. The new course will have 24 students and use two instructors. Class size of 24 students requires two instructors, therefore, the contractor must supply two instructors.
