During an NRC inspection conducted April 27 through May 1,1998, a violation of NRC requirements was identified. In Accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions, NUREG-1600, the violation is listed below:

i License Condition 2.c(4) (Units 1 and 2) of the licensee's facility operating licenses requires, in part, that the licensee fully implement and maintain in effect all provisions of the Commission-approved physical security plan, including amendments made pursuant to the authority of 10 CFR 50.54(p) and 10 CFR 50.90.

Section 3.2.1.4 of the licensee's Industrial Security Plan requires, in part, that a computer controlled intrusion detection system provides an audible and visible alarm signal upon recognizing that an unauthorized access attempt is made using an invalid key card in a card reader.

Contrary to the above, on April 30,1998, the inspector determined that a computer controlled intrusion detectica system failed to provide an audible alarm signal upon recognizing an unauthorized accass attempt using an invalid key card in a card reader.

Specifically, an attempt to unlock card reader doo-No. 502 with an invalid key card provided only a visible alarm signal.

This is a Severity Level IV violation (Supplement III) (50-313;-368/9814-01)

The original design and construction of the computer controlled intrusion detection system provided visual indication on the alarm screen directly in front of the alarm station operator. It cannot be determined from documentation or personnel interviews if the original features also included an audibie alarm signal. During early to mid 1980's the central and secondary alarm station computers were upgraded and badge

' transactions were moved from the alarm screen to a logging screen which did not t

provide an audible alarm signal for the intmsion detection alarm.

The ANO Industrial Security Plan (ISP) contains two sections which describe the intrusion detection alarm differently. One section (3.2.1.4) specifically states that a computer controlled intrusion detection system provides an audibb and visible alarm signal upon recognition that an unauthorized access attempt is made using an invalid key card in a card reader. Another section (5.2.5.1.7) states if authorized conditions are not met (e.g. an unauthorized card [ID Badge) is presented to the card reader), a status message will be generated, displayed, and printed out by a computerized system so that appropriate action can be taken. This ISP section does not describe an audible alarm signal for the unauthorized access attempt.

It is not clear if the audible alarm signal for badge vansactions ever existed or whether it was removed during computer upgrades that occurred during the early to mid 1980's with the appropriate sections of the ISP not being changed accordingly.

Therefore, the reason the intrusion detection system was not equipped with an audible alarm signalis indeterminate.

(2) Corrective steos that have been taken and the results achieved:

Alarm station operators were made aware of the deficient audible signal on the intrusion detection system to increase their sensitivity to the logging screen where badge transactions are recorded.

Other ISP required security computer alarm parameters were tested and verified to be correct.

(3) Corrective steos that will be taken to avoid further violations:

An audible alarm signal will be added to the intrusion detection system by J ' 30, 1998.

Full compliance will be achieved by July 30,1998, when an audible alarm signal will be l

added to the intrusion detection system.
