SUBJECT: NRC INSPECTION REPORT 50-458/98-11
Dear Mr. McGaha:
From May 18-21,1998, a routine inspection was conducted at your River Bend Station reactor
facility. The enclosed report presents the scope and results of that inspection.
The results of this inspection indicate that you have implemented an inservice inspection
program that meets both the ASME Code and NRC regulatory requirements. However, two
oversights were identified during the inspection. The NRC inspector noted an instance where
100 percent of a weld was not examined and a relief request had not been submitted. In
sddition, a quality control nondestructive examiner identified the improper use of a calibration
standard during the preservice and first 120-month inservice inspection interval examinations for
ultrasonic examination of a weld between pipes of different thickness. In each case, the subject
weld was not required to be examined by the ASME Code and NRC regulatory requirements.
However, the oversights indicated a need to improve the review of your inservice inspection
data.
It is our understanding that during the exit meeting 1 May 21,1998, Mr. Dan Pace, Director,
Engineering, stated Entergy Operations, Inc., wa
perform a review of past inservice test data
and relief requests to assure that appropriate
sequests have been, or will be, subtritted by
December 1,1998 (this date corresponds te-
..onths following the end of the first 120 month
inservice inspection interval), and that the rev,ew will be completed by November 1,199E
Additionally, Mr. Pace stated Entergy Operations, Inc., would perform a review of piping
configurations that had a change in pipe thickness to determine if proper calibration standards
were used for the performance of ultrasonic examinations and that the review would be
completed by August 31,1998. Please confirm these regulatory commitments in writing within
30 days of the date of tt.is letter.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and its
enclosure will be placed in the NRC Public Document Room (PDR).
