fl The Model No. 864 shipping container is designed for use as a source chcnger, storage i container, and Ty:pe B shipping container for radiographic sources,in special form. The Modial No. 864 source changer is 6.0 inches in diameter and 9.56 inches high. The ll l package incorporates two handles which protrude from the side of the packagss at its widest point). Three radioactive source assemblies are housed in brass source g tubes. The source tubes are surrounded by uraniun? metal shielding (43 lbs). The uranium a shield assembly is enemd in a carbon steel housing with void space filled with a castable rigic, polyurethane foam. A deck plate above the shield contains three source 8stching f'

I APPROVAL RECORD Model No. 864 Certificate of Compliance No. 9166 l Revision No. 4 By application dated January 9,1998, Amersham Corporation requested renewal of Certificate of Compliance No. 9166 for the Model No. 864 package. Amersham Corporation did not request any changes to the package design or authorized contents.
The staff has reviewed the operating procedures and the maintenance program and found them to be complete and adequate. Two typographical errors were corrected in the reference section.
The Certificate of Compliance has been renewed for a fiva year period which expires June 30, 2003.
