-is based on a number of significant safety violations that.have been aggregated J
into a single Severity Level II _ problem for an inoperable containn,ent recir-culation sump function. The. violations included the failure to perform adequate inspections of the recirculation sump for debris, and the failure to-inspect.
pump suction screens for damage; deficiencies which actually existed and were subsequently-identified by the Resident Inspectors after licensee closecut
' inspections.< The licensee also failed to take adequate corrective action to
- control the,. introduction of debris into the sumps, despite a number of previous
. problems, and. failed to adequately implement corrective action that they committed to'in' response:to two previous Notice of Violations and Proposed Imposition of Civil Penalties _ issued in 1986, which should have identified missing sump 3mmponents required by design specifications..The civil penalty
/ was escalated by'50% because'the NRC identified most aspects of the violations, 50% for[ poor past performance -100% for the prior notice of similar events, and
.another/100% because'some of the deficient conditions may have existed since initia1'startup. The civil penalty was mitigated 50% because of the corrective 0
actions which. included extensive management changes.
