SUBJECT: NRC INSPECTION REPORT 50-382/98-08 AND NOTICE OF VIOLATION
Dear Mr. Dugger:
From March 22 through May 2,1998, an inspection was conducted at your Waterford Steam
Electric Station, Unit 3 reactor facility. The enclosed report presents the scope and results of
that inspection.
During this inspection, three violations of NRC requirements were identified. The first violation
involved the failure to provide adequate guidance to plant operators for the operation of the
emergency diesel generators at no or low electrical loads. The second violation involved the

failure to establish adequate measures to ensure that applicable plant design basis
requirements were correctly translated into surveillance procedure acceptance criteria. The third
violation involved the failure to test thermal overload devices on safety-related valves, as
required by Technical Specifications. In addition, it was noted that this occurrence
demonstrated a narrowly focused scoping of a problem similar to a previously identified
violation. Although the safety consequences were negligible, the regulatory significance of a
condition which forced entry into Technical Specification 3.0.3 was considered high.

These violations are cited in the enclosed Notice of Violation (Notice) and the circumstances

surrounding the violations are described in detail in the enclosed report. Please note that you

are required to respond to this letter and should follow the iristructions specified in the enclosed
Notice when preparing your response. The NRC will use your response. in part, to determine
whether further enforcement action is necessary to ensure compliance with regulatory
requirements.
in accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter, its
enclosures, and your response will be placed in the NRC Public Document Room (PDR).
