June 29, 1978 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PNO-78-127 l

_This preliminary notification constitutes EARLY notice of an event of POSSIBLE safety or public interest significance.

The information presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this.date.

Facility:

Pacific Gas and Electric Company (PG&E)

Diablo Canyon Units 1 and 2, Docket Nos. 50-275 and 50-323 San Luis Obispo County, California

Subject:

AMMONIA INHALATION BY CONSTRUCTION PERSONNEL During air arc cutting of structural steel being performed in the Turbine Building crane rail area, hot slag and sparks fell to the Turbine Building floor and ignited the styrofoam covering of five j

5, gallon liquid amonium hydroxide containers. As a result, three of the five containers broke open and spilled amonium hydroxide.

The fire was promptly detected and extinguished within minutes, but the resulting ammonia vapors caused personnel discomfort due to nii inhalation.

Licensee representatives stated that 29 persons were taken W.

to local hospitals for examination.and that 20 were released immediately.

Nine were admitted for observation.

No structural damage occurred.to i

the Turbine Building.

The licensee notified the local CAL-0SHA. Office, and Region V notified the San Francisco OSHA Office.

The licensee is planning a press release, and the local news media has I

shown interest.

This Preliminary Notification is issued for information only, and no further action by IE is anticipated.

Region V (San Francisco) received notification of this occurrence by telephone from the PG&E Site Superintendent at 8:00 a.m. on June 29, 1978.
