Dear Dr. Bixby:

Thank you for the efforts of your staff during the week of September 11, 1989, when our safety assessment team was at the West Valley Demonstration Project site. Please convey our appreciation to West Valley Nuclear Services as well for their preparations and friendly cooperation.

As we discussed during the exit presentation, the team found no significant shortcomings in three of the five areas examined: welding, environmental moni-toring, and ventilation system design. The team found some problems in the fourth area, quality assurance, although this seems to have improved since the last team assessment in January 1988. The team's most serious concern was in the fifth area, component design and fabrication. Appropriate written procedures for design review appear to be in place, but adherence to the procedures has sometimes been incomplete, and there has been a lack of attention to detail in the design and fabrication process. Our full report, which you can expect to receive in about one month, will explain each of the team's findings in detail.

Since the subject of the team assessment, the Vitrification Facility, is not expected to start hot operations until 1992 at the earliest, we expect that there will be time to make improvements in the design and fabrication process.

None of the team's findings suggest any near-term public safety problem.
