Subject:

Vermont Yankee Response to NRC Bulletin No. 88-10: Molded-Case Circuit Breakers Bulletin No. 88-10 [ Reference b)] requested that addressees take actions to provide reasonable assurance that molded-case Circuit Breakers (CBs), including CBs used with motor controllers, purchased for use in safety-related applica-tions without verifiable traceability to the Circuit Breaker Manufacturer (CBM) perft.m their safety functions.

In accordance with the requirements of the sub-ject Bulletin, Vermont Yankee has initiated a program to investigate the tra-ceability and acceptability of purchased molded-case CBs.

Pursuant to the reporting requirements of the subject Bulletin, Vermont Yankee hereby provides the following written report.

REPORTING REQUIREMENT 1 All holders of operating licenses are required to provide a written report by April 1, 1989 that:

Confirms that only molded-case CBs that meet the criteria of Item 7 of the action requested are being maintained as stored spares for future use in safety-related applications.

b.

Summarizes the total number, manufacturer, model number, and to the extent possible, the procurement chain of those CBs that could not be traced to the CBM in Items 1 and 4 of the actions requested.

For installed CBs, also identify each system in which they are/were installed.

If Item 4 of the actions requested has not been completed by April 1, 1989, due to the sche-dule for tests in Item 3 of the actions requested, this information should be updated within 30 days of the completion of Item 4 to address those additional CBs that could not be traced to the CBM.

c.

Confirms that Items 1, 2, 3, 4, 5, 6, and 7 of the actions requested have been completed or will be completed as requested.

Response

a.

Only molded-case CBs that meet the criteria of Item 7 of the Bulletin actions requested are being maintained as stored spares for future use in safety-related applications at Vermont Yankee. All molded-case CBs whose traceability to the CBM cannot be verified at this time, are being retained in segregated storage at our plant-As of this date, seven (7) of the total 219 identified molded case CB's in stock have been verified to have been traceable to the CBM.

b.

As requested by the Bulletin, Vermont Yankee has identified all molded-case Ces purchased prior to August 1, 1988 that are being maintained as stored spares for safety-related (Class 1E) applications or commercial grade CBs that are being maintained as stored spares for future use in safety-related applications. Our review included CBs purchased directly from CBMs and from other sources. Our review identified 212 CBs that were maintained as safety-related stored spares that are not directly traceable to the CBM at this time.

Of the 212 total breakers, 187 were purchased from Westinghouse Electric

ermont Yankee has issued a Purchase Order (PO) to Westinghouse Company.

at a cost of $15,000 to review records and provide traceability of the 187 breakers where possible. As of this date, because of the large number of inquiries to Westinghouse, Vermont Yankee has not received the results of their review. Vermont Yankee expects to have the results of their review prior to the end of May 1989.

In addition, 15 breakers purchased from General Electric have not be verified as of this date. An additional $15,000 for review of traceability has been quoted in a proposal by GE.

This will be compared to the effort required to search the PO QA requirements on-site per the NUMARC clarifica-

tion of Bulletin 88-10, per Reference d).

In either case, Vermont Yankee expects to have the results of that review completed prior to the end of May 1989.

Traceability of the 10 remaining breakers has not been deter-mined at this time.

breaker traceability effort. Additionally, if required, Vermont Yankee i

will test the untraceable breakers or comply with the requirements of the NUMARC guidance as endorsed by the NRC [ References d) and e)]. to this letter identifies all breakers which have not been traced to the CBM as of this date. The attachment also includes manufac-I turer, model number, quantity, PO number, primary supplier, sub-tier supplier and date purchased.

This attachment will be updated as infor-mation is received and submitted as supplemental information in response to this Bulletin no later than July 1, 1989.

c.

The following provides Vermont Yankee's actions or position with respect to j

each of the seven Actions Requested as identified in Bulletin 88-10.

j 1.

Item 1 of the requested actions was completed in accordance with the Bulletin's schedular request (prior to March 1, 1989).

2.

Item 2 of the requested actions is not applicable to Vermont Yankee.

None of the CBs identified were installed CBs.

3.

Vermont Yankee will complete Item 3 of the requested actions before i

startup from the first refueling outage beginning after March 1, 1989 (approximately October 1990). Vermont Yankee intends to continue to pursue obtaining the required traceability of the Westinghouse and GE breakers. Additionally, Vermont Yankee intends to test any untra-ceable breakers or comply with the recommendations of the recent NUMARC guidance as clarified and endorsed by the NRC [ References d) and e)] which eliminates the need for performing the test.

4/5. At this time, Items 4 and 5 of the requested actions are not appli-cable to Vermont Yankee; however, if they become applicable in the future, Vermont Yankee will implement the requested actions.

6.

Vermont Yankee will comply with Item 6 of the requested actions of the Bulletin.

Information generated while performing the actions requested by the Bulletin will be maintained by Vermont Yankee for a period of five years after completion of all requested actions.

7.

a.

Vermont Yankee is reviewing records to ensure any installation of molded case CBs after August 1, 1988 meet the requirements of Bulletin Request Action item 7.

If any exist and fail to meet the requirements of item 7, these CBs will be treated in accor-dance with Request Action item 5 of the Bulletin.

This review will be completed by the end of May 1989 and summarized and sub-mitted with the supplemental information due no later than July 1, 1989.

Vermont Yankee revised its purchasing process on February 13, l

1989 to require that all electrical equipment be traceable back

)

to the original equipment manufacturer.

In addition, all new I

purchase orders will require that any electrical equipment is I

newly manufactured and not refurbished.

)

l REPORTING REQUIREMENT 2 "All holders of operating licenses are required to rubmit a report that

' summarizes available results of tests conducted in accordance with Item 3 and 5 I

of the actions requested within 30 days after startup from the first and second refueling outages beginning after March 1, 1989.

For CBs that pass these tests, the only information required is the number, manufacturer, model number, and to the extent possible, the procurement chain of CBs tested (summary report format is acceptable).

For CBs that fail these test (s), these reports should indicate the test (s) and the values of test parameter (s) at which the failure (s) occurred, as well as the corresponding manufacturer, model number, and to the extent possible, the procurement chain."

Response

l If any testing specified in item 3 is performed, Vermont Yankee will submit a report summarizing any test conducted in accordance with the criteria iden-tified in the subject Bulletin.

If the testing is not performed, Vermont Yankee will comply with the NUMARC recommendations (Reference d)].

REPORTING REQUIREMENTS 3 AND 4 Bulletin 88-10 Reporting Requirements 3 and 4 are directed to Holders of Construction Permits and are, therefore, not applicable to Vermont Yankee.

CONCLUSION In accordance with the requirements of NRC Bulletin No, 88-10, Vermont Yankee has initiated a program to provide reasonable assurance that molded-case CBs purchased for use in safety-related applications without verifiable tra-('

ceability to the CBM can perform their safety function.

In addition, as requested in Reference c), we will retain any CBs which are identified as unac-ceptable for use in order to support further NRC review of this issue.

l Further, Vermont Yankee will provide a supplement to the response to Bulletin 88-10, identifying the status of all on-going items by July 1, 1989.
