a MODIFICATION NO. 4 TO GRANT NRC-04-88-149 BETWEEN CONFERENCE OF RADIATION CONTROL PROGRAM DIRECTORS, INC.
AND THE U.S. NUCLEAR REGULATORY C0fEISSION This modification is unilaterally issued to extend the grant period of performance identified in Block No. 3.
The Government, hereby accepts your attached letter dated July 31, 1989.
Accordingly, the grant is hereby modified as follows:
1.
Block No. 3, " Grant Period, is extended from July 20, 1988 through July 31,1991."
All other terms and conditions of this grant remain unchanged.
