ROUTINE RADIATION PROTECTION INSPECTION: NRC INSPECTION

REPORT 50-346/98007(DRS) ' AND NOTICE OF VIOLATION

Dear Mr. Wood:

On April 30,1998, the NRC completed a routine inspection of your radiation protection -

. program. The enclosed report presents the results of this inspection.

The inspection was an examination of activities conducted under your license as they relate to

,

radiation safety and to compliance with the Commission's rules and regulations and with the

conditions of your license. The inspection consisted of a selective examination of procedures

and representative records, observations, and Interviews with personnel. Specifically, the

inspection included a review of the as-low-as-is-reasonably-achievabie (ALARA) planning and

controls for the eleventh refueling outage (RFO11), radiation worker performance and the

. maintenance of area radiation monitors.-

) Several recent events occurring at your facility were also reviewed, including: radiation

protection personnel response following resin breakthrough from a purification demineralized;

an unescorted operator entry into a Locked High Radiation Area; unanticipated dose rates in

the annulus during incore detector movement; and several personnel contamination events

. following the removal of steam generator insulation.

Overall, the ALARA planning and controls for RFO11 were good, although some ALARA

planning documentation problems were identified. Radiation worker performance, including job

coverage by RP personnel was also good, with some examples of poor control of contaminated

l

areas and potentially contaminated material. The radiation protection staff effectively evaluated

the changed radiological conditions following the resin breakthrough, and controlled access into

affected areas without incident.

One violation of NRC requirements was identified regarding two examples where your staff

failed to evaluate the potential radiological hazards and institute appropriate controls prior to

performing work. These examples involved the faiiure to: (1) recognize the potential for

transient, high dose rates in the annulus during incore detector movement given known industry

history; and (2) implement the ALARA controls for steam generator work that were specified in

the radiation work permit and discussed in the prejob briefing. Although the potential for a

substantial personal exposure from these events was unlikely, the identified weaknesses placed

workers at increased radiological risk.
