1-2115) requested Toledo Edison (TE) to respond to NRC concerns that TE's Request for Assistance (RFA)/ Document Change Request-(DCR) process does not provide a program for determining design document deficiency trending and for performing root cause analyses and additional reviews of similat areas if trending identified a pervasive problem. The letter requested TE to provide a discussion of how TE's current program determines design document deficiency trending, and when pervasive problems arc;found to' exist, how root cause l
enalyses and additional reviews and actions are performed to assure adequate resolution.
j During the exit meeting for IR 50-346/89015, TE acknowledged that no ongoing program existed for root cause and trending of DCR's and agreed to evaluate the inspector's concern and to consider implementing such a program.
Toledo Edison's evaluation included a review of the DCR process and the. Configuration Management Program (CMP). The CMP identified document discrepancies during-
- i extensive walkdowns that were conducted under this program. These discrepancies are presently being evaluated under a comprehensive on-going effort in which personnel recognize trends during the disposition of these l
discrepancies. The CMP included provisions for-performing root.cause analysis and taking corrective action to prevent recurrence.- Other design document discrepancies are being identified and resolved through the RFA/DCR process separate from the CMP. The current RFA/DCR process for evaluating and' correcting design document deficiencies does not include such provisions.

_ periodic trending of design document deficiencies. When pervasive problems are found to exist this program will require root cause analysis and generation-of corrective actions to prevent recurrence. The necessary elements of this. program are presently being developed and defined. Toledo Edison expects that upon completion of the developmental effort, this program vill be implemented by October 31, 1989.
