To whom it may concern This request is made under the Freedom of Information Act 5 USC 552.

I am requesting access to safety and regulatory records pertaining to five nuclear generating plants in Iowa, Nebraska and Wisconsin.

In particular. I am looking f or any and all records, reports and notices - including inspector notes - on fines levied, safety violationse licensee events or accidents, emergency plant shutdowns or s crams, single component failures, assessments of licensee p e rf o rm an c e , personnel errors, operator errors, security breaches, sabotage incidents and drug use among nuclear workers, from 1980 to present, pertaining to the foJIowing plants: the Duane Arnold Energy Center in Palo. Iowat the Guad Cities Nuclear Generating Station in Cordova, Ill.1 the Cooper Nuclear Generating Station in Brownville, Neb.8 the Fort Calhoun Nuclear Generating Station in Blair, Neb.I and the Lacrosse Nuclear Generating Station in Genoa, Wis.

I am also asking for your list of particularly troubled nuclear plants, or problem plants, and public comments about those plants.

If parts of a document are exempt f rom release under the F0! Act s the rest must be separated and disclosed. Please send all renexempt portions of the records requestede and Justify any deletions by reference to specific exemptions in the F01 Act.

I reserve the right to appeal your decision to withold any materials.

The FOI Act provides for waiver or reduction of research and duplication fees if disclosure could be considered as primarily benefitting the general public. I am a reporter for The Des Moines Register e.nd would use the requested inf ormation f or a newspaper article. Therefore, I seek a waiver of all search and duplication ftes. If you deny that request and the fees exceed $100, please contact me before you fill the request so I can decide whether to pay the fees or appeal your denial.
