dated May 25, 1989 Confirmatory Action Letter 89-08; Restart Commitments Gentlemen:
Pursuant to our letter of May 23, 1989, your Confirmatory Action Letter 89-08 (Reference b), noted your understanding that we would provide the basis for a determination that the issue of apparent material defects associated with the Unit 2 pressurizer penetrations was either not applicable to Unit I or, if applicable, that the issue has been resolved such that the Unit can be safely operated.
Attachment (1) is our Basis for Determination that Unit I can be safely operated even though the issue is applicable to Unit 1.
Attachment (1) fully describes our investigations and the resulting conclusions. We have determined the following:
o Unit 2 has axial cracks in its leaking pressurizer heater sleeves and its leaking pressurizer instrument nozzle.
No evidence of circumferential cracks exists.

o The cracks in Unit 2 have been characterized as Primary Water Stress Corrosion Cracking initiated by high stress imposed in the material during fabrication of the pressurizer.
o An analysis to evaluate the potential for catastrophic failure of the Unit 2 penetrations concluded that the presence of potential axial cracks in the Unit 1 pressurizer heater sleeves or instrument nozzles is not a safety concern.
