DOCKET NOS. 50-445 AND 50-446 HILTI BOLT INADEQUACIES SDAR:
CP-87-068 (SUPPLEMENTAL REPORT)
Gentlemen:
On February 5,1988, TU Electric notified the NRC by final report logged TXX-88174 of several deficiencies involving concrete expansion anchors supplied by Hilti. This report is submitted to clarify a statement made in TXX-88174 regarding the documents being utilized to implement the corrective actions.
TXX-88174 indicated that Field Verification Method (FVM) SWEC-FVM-EE/ME/IC/CS-090 (FVM-90) would be utilized to verify proper torque and the absence of bottomed-out nuts.
However, other FVMs and/or Attribute Evaluation Forms (AEFs) were also used.
Additionally, as stated in TXX-88174, rotating equipment was evaluated under (FVM) CPE-IM-E0-103. Where Hilti bolts were used, according to TXX-88174, bottoming out and torque were to be evaluated using FVM-90.
However, the Hilti bolts were evaluated using non-conformance reports in lieu of FVM-90.
