CONFIRMATORY ACTION LETTER (CAL) # CAL-RIII-89-009 A meeting'was held between Region III and Point Beach management

-on March 23, 1989,- to discuss the results of the NRC-administered requalification examination conducted at Point Beach during the week of February 27, 1989. . Pursuant to this meeting, Region III identified corrective a'ctions in the confirmatory action letter of March 28, 1989.

The following actions have been completed in accordance with the CAL:

1. The five individuals who failed the'NRC examination were immediately removed-from the watch bill and have not performed any licensed duties  ;

since the exam.

2. Accelerated remedial training for these five individuals has been completed. The training required for each person was based on an analysis of individual. performance on the exam-(e.g., integrated operations, I&C, E0Ps, etc.); it took the form of either formal classroom training and quizzes, or closely monitored and graded individual assignments.

In addition, a complete written examination meeting the requirements of NUREG-1021 was developed, and provided to Region III for review and approval. This exam was administered by Region III on May 17, 1989.

However, one individual was unable to take the examination due to a personal injury sustained two days before the exam date that required hospitalization. We will work closely with Region III personnel to formulate a new plan for examining this individual with an NRC-

.. administered exam. The other four passed the exam as determined by both NRC grading and PBNP grading. In accordance with the CAL, these operators have been reinstated to their watchstanding duties. Mr. Burdick concurred with this action in a telephone conversation with Dr. Bruno on May 19, 1989.

3. The examination developed for May 17 was provided to Region III for review and approval on May 3. Personnel from Point Beach personally delivered the proposed exam material, and remained at the region for two days to finalize the exam. The material was accepted with minor modification, and

. formally approved by the lead examiner upon arriving on site to conduct the examination.

4. Enhanced training was provided to all licensed personnel during the continuing training cycle between April 10 and May 19. The training focused on rod control and plant response, with emphasis on the topics of questions one through eight of Part A of the NRC exam from February 27.
5. Copies of the quizzes, and graded answers, associated with the enhanced training mentioned above were provided to the Region III offices. In all cases, the material was mailed to the NRC within one week of administration of each requal group.
6. With the exception of the injured senior operator's examination, all actions identified in the CAL have been completed.

Until the NRC has graded the individual's exam, and we are satisfied that he is capable of performing licensed duties, he will not function as a senior reactor operator at Point Beach.
