TVA letter to NRC dated May 5, 1989, "Sequoyah Nuclear Plant (SQN) Unit 1 - Balance of Unit 1 Cycle 3, 90-Day Inservice Inspection (ISI) Report" This letter provides the results of TVA's evaluation of six WRs that were unavailable for inclusion into the SQN unit 1 cycle 3, 90-day ISI report (Reference 2). contains a description of TVA's evaluation process and the additional activities that will be required to confirm the complete status of each WR. contains an itemized list of the equipment / system affected, the work requested by each WR, and the current status. Enclosure 3 contains TVA's commitment regarding the additional work activities described in enclosure 1.
TVA's evaluation confirmed that all six WRs were in a work-complete status at the time the WRs were misplaced. This provides a high level of confidence that the work activities were completed as requested and that no safety concerns exist.

' Work requests-(WRs) A292730, A300038, A532451, AS48495, A562952, and B106725 have been evaluated in accordance with American Society of Mechanical Engineers (ASME)Section XI requirements and have been processed in accordance I
with the requirements of SQN Standard Practice SQM2, " Maintenance Management System.".TVA has issued replacement WRs (identical WR numbers) to inspect the component / equipment affected by the original WR and confirm that the work was l:
completed as originally requested. This validation process to confirm the work-complete status will occur for five of the six WRs.
Mork -*equested under WR B106725 was found to be documented as complete under a related workplan (IWP] M270-01).
The work activity requested by the WR was repeated in the WP following the recorded WR performance. The ASME Section.X1 summary report from WP M270-01 was included in the partial unit 1 cycle 3, 90-day inservice inspection (131) report dated February 21, 1989. WR B106725 is curreni.ly being placed on microfilm and will be sent to SQN's Document -
Control Unit for permanent storage ("CX" status). No further action is required for WR B106725.
j-The remaining five replacement WRs have been written, planned, and' approved (by SQN Quality Assurance and the Authorized Nuclear Inspector)'and have been placed in a Priority 6 (PA-6) status. Priority 6 means these WRs will be issued for work during the next refueling outage (Modes 5 or 6).
The Priority 6 status is necessitated by the fact that the component / equipment is located in areas that are inaccessible during power operation. Work that was previously requested by the original WR will be confirmed, and all required nondestructive examinations will be reperformed as required.
