requests renewal of Quality Assurance Program, Docket Number 71-0181, Revision number 3.
Please be advised that as of November 10, 1987 we ceased license activities under the name of Consolidated X-Ray Service Corporation and continued under the name of Century Inspection, Inc. (License No. 42-0856-62 Amendment No. 27).
Attacaed are two copies of the revised program and a check for the required fee of $150.00.
