4 4.4 Monitoring Program Problems and Corrective Action 4.4.1 Milk Sample Trojan Technical Specifications require milk samples be collected at least once per 15 days during all months of the year except December, January, and February. Contrary to this requirement, milk samples in 1988 were collected 21 days apart on three separate occasions. The cause of this event was failure to follow procedure. The nilk sampling instruction specified that the samples be taken semi-monthly, while the overall instruction specifying all sample frequencies stated that the samples be teken every 15 days. The term semi-monthly was interpreted to mean twice per mcuth versus every 15 days. The milk samples usually were collected every other week on Mondays. To avoid sampling three times in some months, the Health physics Laboratory Supervisor decided to skip an extra week when necessary to keep the sampling frequency at two times per month. This resulted in the three sampling intervals of over 15 days during 1988.

To prevent recurrence of this problem, the instructions for milk sampling are being revised to provide clearer guidance on required periodicity.

During 1989, the required sampling intervals for milk have been met.

4.4.2 Drinking Water Samples During 1988, the monthly composite samples of drinking water from the automatic sampler at the upstream location were supplemented with grab samples due to a sampler malfunction. The automatic sampler failed to collect a full and complete sample due to a drop in the water level at the sampling location between field checks of the equipmeat. During 1988, the field checks were performed at approximately two week intervals.

i l To prevent recurrence of this problem, the frequency of field checks of the water sampler was increased to approximately once per week. In addition, the water sampling procedure is being modified to list the requirement that l sampler malfunctions be reported in the annual report.
