April 10,1998 EN 98-024 OFFICE OF ENFORCEMENT NOTIFICATION OF SIGNIFICANT ENFORCEMENT ACTION Individual:
John Boschuk, Jr.

Sublect ORDER PROHIBITING INVOLVEMENT IN NRC-LICENSED ACTIVITIES This is to inform the Commission that an Order Prohibiting involvement in NRC-Licensed Activities was issued on April 10,1998 to John Boschuk, Jr. The action was based on an inspection and Ol investigation which concluded that Mr. Boschuk deliberately violated NRC requirements and otherwise committed willful violations of NRC requirements, while serving as an agent for, and a consultant to, J&L Testing Company, Inc., a materials licensee of the Commission. Concurrently on this date, the NRC is issuing an Order revoking the license issued to J&L Testing Company, Inc. The Order details violations of NRC requirements committed by Mr. Boschuk, including making unauthorized transfers of byproduct material, making materially inaccurate statements to the NRC, and destroying records required to be maintained by J&L Testing, Inc.'s license The Order removes Mr. Boschuk from engaging in NRC-hcensed activities for a period of five years. In addition, for that same period of five years, Mr. Boschuk shall provide a copy of the Order to any prospective employer engaged in NRC-licensed activities, provide notice to NRC the first time following the prohibition that he engages in NRC-licensed activities, and cease activities if he is currently involved in NRC-licensed activities.
It should be noted that the individual has not been specifically informed of the enforcement action. The schedule of issuance and notification is:

The individual has twenty days from the date of the Order in which to request a hearing.
