Joseph M. Farley Nuclear Plant - Units 1.and 2 Notification of Removal From Licensed Duties As requested during the exit conference on June 29, 1989, this letter outlines the process used at Farley Nuclear Plant to ensure a licensed operator, who has failed his/her annual retraining, does not perform licensed duties until remedial training and reexamination has been successfully completed.

When a failure has been identified, the Operations Manager is notified immediately and'the individual is removed from licensed duties. A follovup training memorandum is sent to the Operations Manager and the individual licensee to inform them of the examination failure, deficient areas and a reminder that he/she is not to perform licensed duties.

The Operations Manager also initiates a Notification of Removal From Licensed Duties Form (attached). This form must be signed by the individual licensee acknowledging failure of the exam and his/her restriction'from performing licensed. duties. This memo also defines the licensed duties that the individual may not perform and the shift positions he/she may not fill until remedial training has been successfully l

completed.

The following definition of " licensed duties" vill be used in reference to

. personnel not passing the annual requalification examination.

1.

Manipulation of reactor controls.

2.

Manipulation of controls which affect reactor power level.

3.

Approval of temporary change notices as an SRO.

4.

Direct supervision of licensed operators in the performance of 1 and 2 above.

5.

Signing LCO Status Sheets as Shift Foreman Operating or Shift Supervisor..

6.

Personnel who have been removed from licensed duties as a result of not passing the annual requalification examination vill not fill the following shift positions:

- Unit Operator

- At the Controls Operator Shift Supervisor
