Subiect ORDER PROHIBITING INVOLVEMENT IN NRC-LICENSED ACTIVITIES l
1 This is to inform the Commission that an Order Prohibiting involvement in NRC-Licensed Activities was issued on April 10,1998 to Lourdes T. Boschuk. The action was based on an inspection end Ol investigation which concluded that Ms. Boschuk deliberately violated NRC requirements and otherwise committed willful violations of NRC requirements, while serving as president of J&L Testing Company, Inc., a materials licensee of the Commission. Concurrently on this date, the NRC is issuing an Order revoking the license issued to J&L Testing Company, Inc. The Order details violations of NRC requirements committed by Ms. Boschuk, including making materially inaccurate statements to the NRC and destroying records required to be maintained by J&L Testing Company, Inc.'s license. The Order removes Ms. Boschuk from engaging in NRC-licensed activities for a period of five years. In addition, the Order requires that Ms. Boschuk provide a copy of the Order to any prospective employer engaged in NRC-licensed activities during the prohibition peiiod, provide notice to NRC the first time following the prohibition that she engages in NRC-licensed activities, and cease activities if she I
is currently involved in NRC-licensed activities.
It should be noted that the Individual has not been specifically informed of the enforcement action. The schedule of issuance and notification is:

The individual has twenty days from the date of the Order in which to request a hearing.
