Attachment A - STATEMENT Of WORK,Section IV entitled " Reporting," paragraph A.,
DELETE the first sentence in its entirety and SUBSTITUTE the following sentence in lieu thereof:
"A.
.The. State will provide the Commission with an annual report of all off-site analysis with comparisons of'similar analyses by the respective licensee by September 1 of each year."
Also, the third sentence in paragraph A. is revised to read as follows:
"In the event that some result are not available by the reporting period, the report shall be submitted noting' ind explaining the reasons for the missing resul ts."
All other terms and conditions remain unchanged, including the obligated' amount of $83,500.00.
