On April 17. 1998. Mr. Paul Fredrickson provided you with the results of a

U.S. Nuclear Regulatory Commission (NRC) inspection at your Farley Nuclear

Plant (FNP).

The onsite inspection was completed on March 25. 1998.

The

enclosed report presents the results of that inspection.

The purpose of the

inspection was to follow up on the findings and conclusions from an

investigation conducted by the NRC Office of Investigations (OI) and

documented in a report issued on February 9. 1998. A copy of the synopsis

from the report is enclosed.

OI initiated the investigation to determine whether a contract welder

deliberately violated NRC requirements while making a socket weld in piping in

Unit 1 of your Farley Plant on March 5.1997.

As indicated in the synopsis.

01 concluded that the welder intentionally made safety-related weld repairs

without obtaining a required weld repair procedure during the March 1997

event.

A similar event involving the same contract welder occurred in 1993.

Based on the results of the OI investigation and the NRC's March 23-25. 1998

inspection, a violation of NRC requirements occurred.

The violation involved

the contract welder's failure to document a deficiency and his subsequent

deviation from the work sequence by making a base mateial weld repair to the

pipe without a properly authorized repair plan.

As a result, the weld was not

in compliance with the ASME Boiler and Pressure Vessel Code.

As indicated in

the enclosed report, we have determined that you identified this violation and

implemented appropriate corrective action.
