Violation of National Pollutant Discharge Elimination System (NPDES) Permit Pursuan', to Section 3.2 of Appendix B to Facility Operating License No. NP'/-1 for the Trojan Nuclear Plant, the attached report is forwarded describing a violation of the NPDES Permit during April 1989. The vio-lation involved exceeding the limit for flow from the settling basin.

Enclosed is the NPDES report for t Trojan Nuclear Plant for April 1989. Also enclosed are data required to be submitted in addition to that on standard NPDES forms. The period in which cooldown limits applied is high lighted in yellow.

The plant was shut down on April 6 for the annual refueling outage which is scheduled for completion in mid-June.

The settling basin flow limit of 80,000 gallons per day was exceeded on April 14 with a recorded flow of 88,100 gallons. The high flow was due to the inadvertent actuation of an auxiliary cooler on the condenser air ejector system. This cooler discharges service water (river water) to the settling basin when in use. The high flow for April 14 was identified when the daily settling basin flow was determined at 0015 on April 15. The

[ source of the excessive flow was quickly identified and isolated.

Water from the circulating water system was drained to the recreation lake from 0515 on April 9 to 1711 ea April 10.

Draining of the circulating water system was performed to allow for inspection and repair of the system during the refueling outage. All required samples were taken and were within specification. Your office was notified, as required, by telephone prior to this event.

The main outfall pH monitor remained out of service during the entire month. Grab samples of the main outfall were taken and f analyzed every eight hours to verify compliance with NPDES pH j limits. This monitor has been out of service since December 31, 1988 because of a failed sample pump. Use of a replacement pump is pending approval of a temporary modification to allow installation of the pump which is of a different capacity than the

' original failed pump.

The sewage treatment plant has operated well within limits during the month. Approximately 617,000 gallons of sewage was treated at the on-site treatment plant, and 430,900 gallons was transported to the St. Helens, Oregon sewage treatment facility.
