Dear Sir:
We wish to renew our current Quality Assurance Program Approval Certificate Number 0128 which expires on September 30, 1989.
Except for the changes in the attached Company Table of Organization indicating the current chain of responsibility, our Quality Assurance Program will continue to be maintained and executed as required by 10 CFR Part 71 to return (ship) packages of radioactive materials back to the original supplier.
Our written Operating and Emergency Procedures require us to use industrial radiography as a tool to assure safe conditions of refinery equipment and to comply with all NRC regulations governing industrial radiography.
M. M. Shendrick, Chief Inspector on Special Assignment, is Radiation Safety Officer'(RS0) and has the responsibility for the Radiation Safety Program as authorized by NRC License No. 13-00155-10.
A license amendment application submitted August 17, 1989 to the NRC Region III Office (Control No. 87846) is pending with a request to approve M. J. Berna, Chief Inspector, to replace M. M. Shendrick, who is retiring at the end of the year.
All required material and personnel records, manufacturer certificate files, reminders, notices and communications pertaining to radiation safety and equipment are the responsibility of Administrative Assistant Shirley Manning. There are written instructions to

MacDonald, Chief Page 2 The individual radiographic sources'that are currently used at our facility are purchased-from Amersham Corporation (formerly Technical Operdions Inc.), holders of NRC Quality. Assurance Program Approval No. 0040. All new replacement sources are shipped to us in NRC and D0T approved containers certified for safe handling of the strongest I
sources that we obtain (see attachments).
-Our Operating and Emergency Procedures detail the step-by-step procedures for changing sources and servicing the supplier's containers.- The container with the weaker source is sealed, radiation
-levels measured and recorded, necessary shipping 1abels attached and shipping papers prepared. Only then is the container shipped back to the source supplier via NRC approved carriers. Source supplier sends a receipt acknowledging return of our weak sources. All handling and monitoring of the sources is performed by a trained and qualified radiographer under the direction of the RSO. Records of receipts, surveys, tests, source inventories and disposals are maintained by Shirley Manning for-periods of time per NRC regulations. Our Radiation Safety Program is monitored quarterly and audited annually by the RSO.
Accompanying this renewal application is a check for $150.00, a fee pursuant to 10CFR Part-170.31.
