POINT BEACH NRC INSPECTION REPORT 50-301/98005(DRS) AND

Dear Mr. Patuiski:
On March 9,1998, the NRC completed an inspection of your Point Beach Nuclear Plant, Unit 2
facility. This inspection focused on the weld procedures used during the Unit 2 steam generator
replacement project conducted in the fall of 1996. At the conclusion of the inspection, the

findings were discussed with those members of your staff identified in the enclosed report.
Based on the results of this inspection, we identified four violations of NRC requirements.

. Three of the violations pertained to weld procedures that either did not meet American Society
I
of Mechanical Engineers Code (Code) requirements or did not prescribe activities performed.
l
The fourth violation involved several examples of errors in the documentation for welds
'
completed during the steam generator replacement project. The documentation errors
'
hampered your staff's and the inspector's efforts to verify that the Code requirements had been
met for the affected welds. Although the no immediate safety concern exists for the integrity of
the affected welds, the required service properties (impact toughness) have not been
adequately demonstrated in accordance with Code requirements.
The circumstances surrounding these violations and areas of concem are described in detail in
the enclosed report. Please note that you are required to respond to this letter and should
follow the instructions specified in the enclosed Notice of Violation when preparing your

response. The NRC will use your response, in part, to determine whether further enforcement
action is necessary to ensure compliance with regulatory requirements.
in addition, the problems identified by the inspector demonstrated that the recent audits of the

welding procedures and documentation, conducted by both your staff and the welding
l
contractor, lacked rigor and were ineffective. Therefore, in your written response to the
'
attached Notice of Violation, please amend the information contained in your October 16,1997,
letter, and address the root causes and corrective actions for the ineffective welding audits
discussed herein.
