Dear Dr. Orton:
Based on the information and data submitted in your application dated May 17, 1991, with enclosures thereto, we have approved your request for a custom SSD registration certificate (Theratronics' certificate no.
NR-687-D-107-S). In addition, we conclude the modified Theratron 780-19 total body irradiator is acceptable for licensing purposes in accordance with the conditions of the enclosed registration certificate. A copy of this certificate has been forwarded to our Region III office. The registration certificate is conditional upon Harper Hospital providing a external radiation survey of the device prior to first treatment or within 90 days of the date of this letter, whichever comes first.
Please read over the registration certificate in its entirety and notify 1 us immediately if there are any errors.
