Examination Summary Examination administered on August 23, 1989 (Report No. 50-440/0L-89-02))
One written requalification examinations was administered to a Senior Reactor Operator (SRO), who did not pass the initial written examination on June 29, 1989.
Results: The.SRO passed the retake for the written requalification examination. The parallel grading between the facility and NRC of examination results were within the guidance provided by NUREG-1021, ES 601.

' Examination Test Bank Material needs.to be revised to remove "True and False" type questions from bank. Until more definitive guidance is pro.vided in NUREG-1021, ES-601, the policy provided in ES-401 should be used to develop the question' format. The facility accepted this Comment.
