During the Nuclear Regulatory Commission (NRC) inspection conducted on
,1 February 13-17, 1989, a violation of NRC requirements was identified.

accordance with thb " General Statement of Policy and Procedure for NRC.
Enforcement Actions," 10 CFR Part 2, Appendix C (1989), the violation is l
listed below:
j 10 CFR Part 50, Appendix B, Criterion V, " Instructions, Procedures, and Drawings," require that activities affecting quality shall be prescribed l
l by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and that the activities be accomplished in accordance with these instructions, procedures or drawings.
Contrary to the above, activities affecting quality were not accomplished in accordance with general procedures and drawings for control of l
Inservice Inspection (ISI).
On February 15, 1989, the NRC inspector observed a liquid penetrant examiner erroneously conducting an examination on Chemical and Volume Control System Class I weld, CVC-1090 as shown on drawing MSG-0008-C, instead of the weld identified on the drawing as requiring examination, weld CVC-1091.
A subsequent investigation of this examiners activities conducted on February 16, 1989, revealed that on January 28, 1989, two Class 1 socket welds in the Chemical and Volume Control system, welds CVC-1103 and CVC-1104, were i
liquid penetrant examined instead of required welds CVC-1101 and CVC-1102 as shown on drawing MSG-0008-C.
As a result of these erroneous examinations, the welds identified as requiring examination would not have been examined.
In addition, the licensee conducted a partial surveillance of the liquid penetrant examination of incorrect welds f
CVC-1103 and CVC-1104 on January 28, 1989, as documented in Monitor Report No. 555-89-24, while the liquid penetrant examinations were in progress but this surveillance failed to identify that the welds which were being examined were not the welds that required examination.
This is a Severity Level IV violation (Supplement I).

DPR-77 and DPR-79 Pursuant to the provisions of 10 CFR 2.201, Tennessee Valley Authority is hereby required to submit a written statement or explanation to the Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, DC 20555, with a copy to the Regional Administrator, Region II, and a copy to the NRC Resident Inspector, Sequoyah, within 30 days of the date of the letter transmitting this Notice.
This reply should be clearly marked as a " Reply to a Notice of Violation" and should include [for each violation]:
(1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved.
Where good cause is shown, consideration will be given to extending the response time.
If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken.
