Task Order No. 4, Entitled " Data Entry For General License Data Base"., Under Contract No. NRC-39-89-133 This confirms verbal authorization provided to you on January 31, 1989 to commence work under.the subject Task Order with a temporary ceiling of
$6,000.00.
In accordance with the task order procedures of the subject contract, this letter definitizes Task Order No. 4.
This effort shall be performed in accordance with the enclosed Statement of Work and the Contractor's proposal for this Task Order No. 4 dated January 31, 1989, incorporated herein by reference.
Task Order No. 4 shall be in effect from February 1, 1989 through January 30, 1990 with a total cost ceiling of $35,481.00.
The amount of
$33,006.00 represents the total estimated reimbursable costs and the amount of $2,475.00 represents the fixed fee.
The total cost ceiling of $35,481.00 is inclusive of the $6,000.00 verbally authorized as discussed above.

/ NMSS/INMS Requested Period of Performance: From 02/01/89 Through 01/31/90 TAC #: A64075 STATEMENT OF WORK The contractor shall provide data entry support for the purpose of entering information about neneral licensees and the device (s) they possess into the General License Data Base (GLOB).
Data shall be entered by the data entry operator using the General License Data Base, a RAMIS based sy. stem, which runs under TSO.
The volume of data is estimated to be approximately 24,000 records, with each record containing an average of 500 characters.
The contractor shall prepare the data for entry, enter the data and verify e
input prior to performing the online update function.
The level of effort for this task order is estimated to be 1776 hours0.0206 days <br />0.493 hours <br />0.00294 weeks <br />6.75768e-4 months <br />.
All work shall be performed using CRT terminals located in One White Flint florth in Rockville, Maryland.
