In accordance with the task order procedures of the subject contract, this letter definitizes Task Order No. 5.

This effort shall be performed in accordance with the enclosed' Statement of Work and the Contractor's proposal for this Task Order No. 5 dated January 31, 1989, incorporated herein by reference.

Task Order No. 5 shall be in effect from February 1,1989 through January 31, 1990, with a total cost ceiling of $34,574.00.

The amount of

$32,162.00 represents the total estimated reimbursable costs and the amount of $2,412.00 represents the fixed fee.

The total cost ceiling of $34,574.00 is inclusive of the $4,000.00 verbally authorized as discussed above.

Data Entry Support For SINET i

TASK ORDER #5 BACKGROUNO:

An a result of the development of a Safety Information Network, the Nuclear Regulatory Commission (NRC) has a need for data entry support for input of Licensee Event Reports (LERs) into an automated system residing at the NIH computer facility in Bethesda, Maryland.

STATEMENT OF WORK:

The contractor shall perform the data entry work at the NRC site in Bithesda, Maryland, using IDMS/R ADSO data input screens. This system resides at the NIH computer facility in Bethesda, Maryland. A source document for the data entry is presented in Attachment II (the data elements that will probably require entry have been circled).

It will consist of approximately 16 c'sta elements plus text (the abstract) averaging about 15 eighty character lines.

The "backfit" portion of the job shall consist of approximately 225 source documents. Simple instructions and training will be' provided to the data entry person at the NRC site. After an initial brief break-in period, it is i

anticipated that each document will take about 15-20 minutes to complete.

Input quality assurance and any resulting corrections shall be j

the responsibility of the contracto.T.

In addition to backfit LER data, NRC receives about 10-15 LERs daily.

Pressure setpoint testing of the 14 Target Rock Hodel 7567 F pilot operated safety relief valves (SRVs) during the first refueling outage revealed that only four SRVs lifted within + lt of the nameplate setpoint.

In order to be considered'operabTe in i

accordance with Technical Specification 3.4.2 the valve must lif t i

within.+ 11 of its setpoint value.

conditions and no release of radioactive material associated with

this event.

Corrosion bonding'between the Stellite pilot valve disc and Stellite seat and labyrinth seal clearance restriction I

is considered to have caused the over 31 cetpoint drif t of the SRVs (see figures 1 and 2).

All 14 SRVs were replaced with spares that had been setpoint certified in 1986.

Mechanical Engineering in. conjunction with the Boiling Water Reactor Owner's Group (BWROG) is evaluating a pilot disc material change to j'

reduce the effects of corrosion bonding between the pilot disc and sea t.

SRVs which identify labyrinth seal induced friction yill be reworked to maintain the design clearance between the pi-lo Q od and liner.
