DOCKET NOS. 50-445 AND 50-446 GAMMA-METRICS NEUTRON FLUX MONITORING

'SDAR:.CP-89-003 (FINAL REPORT.)

1 Gentlemen:

On February 15, 1989, TU Electric verbally notified the NRC of 'a deficiency-involving Gamma-Netrics supplied cable assemblies for post-accident neutron flux monitors. 'The last report on this issue was loggea TXX-89361, dated June 2, 1989. After further evaluation, it has'been concluded that this

~ deficiency meets the criteria for deportability pursuant to 10CFR50 55(e) and

-the required.information follows.

Description of Deficieney Gamma-Metrics issued a 10CFR Part 21 notification that addressed possible leaks in soldered connections and threads of their neutron flux monitoring system cable assemblies.

~A soldered joint on.the cable assembly metal hose failed to hold pressure during a generic qualification test performed at elevated temperatures.

Vendor records revealed that CPSES had four cable assemblies of a similar design.

TV Electric. initiated a nonconformance report and subsequently confirmed that

'the identified cable assemblies were of a similar design to those identified inLthe' Gamma-Metric's notification.

Safety Significance

.At CPSES, these cables are utilized in instruments that are Regulatory Guide 1.97 monitors and provide information.to indicate whether plant safety functions are being accomplished during mitigation of an accident.

Failure of

.these cables could have adversely affected operator response to' post accident conditions and represents a potentially significant deviation from performance specifications.

As a result, this issue meets the criteria for deportability-

.pursuantEto 10CFR50.55(e).

Correctiv'e Actions The Gamma-Metrics neutron flux monitors consist of two assemblies; the cable.

assembly and the detector assembly. Requisitions were issued on February 29, 1989, to return four cable assemblies ~to the vendor for inspection, testing and rework. The cable assemblies were. inspected, tested and repaired by the vendor at their facility and were witnessed by CPSES OA representatives.

The cable assemblies were returned to CPSES and installed by the vendor-for Unit 1.

The Unit 2 cable assemblies were returned to warehouse storage.

Five (5) of the detector assemblies, including one spare, have been returned to Gamma-Metrics for inspection, testing and rework. Corrective action for the Unit I assemblies will be completed prior to Unit 1 fuel load, and Unit 2 detector assemblies prior to Unit 2 fuel load.
