INTRODUCTION This submittal complies with the format described in Regulatory Guide 1.21, " Measuring, Evaluating and Reporting Radioactivity in Solid Wastes and Releases of Radioactive Materials in Liquid and Gaseous Effluents from Light-Water Cooled Nuclear Power Plants", Revision 1, June, 1974.

The following information is included as Tables to this report:

A summary of the gaseous and liquid effluent releases for the report period.

Where "0.00E+00" is used, it denotes the less than detectable level for the given isotope.

Noble gas effluents are indicated for release point number 3:

Unit 2 South Stack.

Although Unit 2 I

had not yet undergone fuel loading or initial criticality at the time these isotopes were released, the Unit 2 reactor vessel shares a common fuel floor l

with Unit 1.

Noble gaFas released during Unit 1 shutdown and refueling activities and Unit 2 ventilation startup (1st quarter 1989) were conducted through the Unit 2 effluent passage.

hithough these fission gases are actually attributable to Unit 3 t

activities conducted under License NPF-39, they are reported for their actual release point from Unit 2 South Stack.

Composite particulate air samples counted for beta emitters (eg. Sr-89, Sr-90) are submitted to an offsite vendor laboratory for analysis.

Since data for the second quarter particulate air samples had not yet become available at the time this report was prepared, it will be necessary to submit an addendum in the future if second quarter particulate air samples results are reported >LLD.

a Since Limerick Technical Specification limits for liquid and gaseous effluent releases are prescribed in terms of quarterly and annual offsite doses, " percent technical specification limit" is entered as 0.00E+00 below effluent release rates and quantity totals.

A summary of solid waste dispositioned during the report period, to include: total activity shipped by waste type and an estimate of the error in the reported totals; the estimated composition of each type of waste by isotope; the number of shipments, mode of transportation, destination, type of container, total container volume, and solidification agent.

RESIDUAL HEAT REMOVAL (RHR) SERVICE WATER RADIATION MONITOR FAILURES l

During the first half of 1987, the "A" and "B" RER Service Water l

Radiction Monitors were declared out-of-service due to indications of low lsamplo flow.

An investigation of the problem revealed the accumulation lof corrosive materials on the walls of the piping.

Several factors were attributed to the corrosion:

river water, which is highly aggressive to carbon steel, is used

to supply service water.

The incorporation of new pipe material (from carbon steel to 41inless steel) and reduced bendings in the pipe lines (to facilitate stem flushing) was evaluated as the long-term solutions to redeposition corroaive products in the RHR Service Water Radiation Monitor sample 4

)iinO3.

The thirty (30) day action described in Technical Specification i Tablo 4.3.7.ll-lb, Action 101 was exceeded because the corrective maintenance of replacing sample piping could not physically be performed i

lin th$ time specified while observing all applicable station i

indministrative controls.

During the second half of '87 and the first half of '88 the monitor remained inoperative and Action Statement (101

,waa observed.

The modification was completed during the last quarter of j

l'88 ct which time the A, B and D monitors were declared operable.

i During the first quarter of '89 the A, B and D monitors were again d:clared inoperative due to flow adjustment problems.

This period of I

inoperability exceeds Technical Specification 3.3.7.11 and Action Stotcrent #101 was initiated.

All four monitors were returned to service beforo the end of the first quarter and maintained operable for the-remainder of the report period.

SERVICE WATER RADIATION MONITOM During the second quarter of 1989 the Service Water Radiation Monitor 4

wn3.rcmoved from service for corrective maintenance and pipe replacement

cioilar to that of the RHR Service Water Radiation Monitors, described in i

Revision 7 Two milk sampling stations selected for the Radiological Environmental Monitoring Program (REMP) went out of business in the l

third and fourth quarters of 1988.

One station was not replaced within thirty days resulting in a violation of Technical Specifications.

(LGS LER 89-30)

Upon subsequent review, the REMP was modified to add another farm which met the requirements of l

Table 3.12.1-1 of LGS Technical Specifications.

This addition occurred.in January, 1989.

Procedures and specifications were reviewed with the sample collector to assure prompt replacement of f

Technical Specification stations.

The O.D.C.M. was revised to include the new sampling stations.

One l

TLD control station, (18Gl-C) omitted in the previous revision, has l

been included in revision 7.

Fish and food product sampling stations, inadvertently omitted in the previous revision, have now j
