1.0 DESCRIPTION

OF QUALITY ASSURANCE PROGRAM FOR TYPE "B" QUALITY SHIPMENTS OF RADIOACTIVE MATERIAL:

1.1 ORGANIZATION

1.1.1 SAM-SON INSPECTION & TECHNICAL SERVICES, INC.

pursuant to 10-CFR 71.12 and 10-CFR 71.51, sub-paragraph (a) is responsible for a Quality Assurance Program for Type "B" quality shipments and transportation of radioactive material.

Fabrication and design of Type "B" packages are not included.

1.1.2 The organization structure in Part I of the Radia-tion Safety Manual is applicable, a.

The Radiation Safety Officer (RSO), is respon-sible for administration of the program, train-ing of personnel, procurement of manufacturer's certifications, maintaining packaging and shipping documents, retention of records and program audits.

Radiographer are responsible for handling, storage, shipping and tratisporting radioactive packages in accordance with the procedures in Part II of the Radiation Safety Manual; and they are responsible for inspecting, surveying, and maintenance reports in accordance with the procedures in Part II of the Radiation Safety Manual.

1.2 OUALITY ASSURANCE PROGRAM:

1.2.1 Radiographer are trained in their responsibilities above according to Part I of the Radiation Safety Manual.

1.2.2 The Quality Assurance Program will be viable, requiring management approval, and the program encompasses specific requirements stated in the package approval for use, and general radiation safety and shipping requirements.

1.2.3 Manufacturer's certifications will be requested

PART III - QUALITY ASSURANCE PROGRAM

1.0 DESCRIPTION

OF QUALITY ASSURANCE PROGRAM FOR TYPE "B" QUALITY l

SilIPMENTS OF RADIOACTIVE MATERIAL:

1.2 QUALITY ASSURANCE PROGRAM:

1.2.3 and maintained on all N.R.C. Type "B" packages manufactured and procured after January 1, 1979.

1.3 DOCUMENT CONTROL:

1.3.1 Packaging, shipping, inspection and handling instruc-tions from the manufacturers shall be maintained for each model Type "B"

package.

1.3.2 The Radiation Safety Officer shall insure that the shipment and transportation of Type "B"

packages is conducted in accordance with the above documents.

1.4 HANDLING, SHIPPING, TRANSPORTATION AND STORAGE:

1.4.1 Radiography personnel will handle, ship, transport and store radioactive packages in accordance with the procedures in Part II of the Radiation Safety Manual.

1.4.2 Radiography personnel will determine the completeness of package inspections and presence of manufacturer's certificates before shipment.

1.5 INSPECTION, SURVEYS AND MAINTENANCE REPORTS:

1.5.1 Inspection, surveys and maintenance reports on Type "B" packages will be made in accordance with Part II of the Radiation Safety Manual.

1.5.2 Quarterly inspection and maintenance of Type "B" packages will be performed by the Radiation Safety Officer as outlined in Part I of the Radiation Safety Manual.

1.6 QUALITY ASSURANCE RECORDS:

1.6.1 Utilization logs, shipment records, package surveys, and quarterly inspection records will be maintained as specified in Part I of the Radiation Safety Manual

1.6 QUALITY ASSURANCE RECORDS:

1.6.2 Training records are maintained in accordance with Part I of the Radiation Safety Manual.

1.6.3 Equipment and package procedures and instructions are maintained by the Radiation Safety Officer.

1.7 AUDITS

1.7.1 Quarterly audits of the entire Radiation Safety Program are performed in accordance with Part I of the Radiation Safety Manual.

1.7.2 The quarterly audits include the necessary checks on the Quality Assurance Program as outlined in Part I of the Radiation Safety Manual.

1.7.3 Any deficiencies revealed by the audits are corrected in accordance with Part I of the Radiation Safety Manual.

f 1.8 AUDIT RECORDS:

1.8.1 Records of quarterly audit results will be main-tained by SAM-SON INSPECTION & TECHNICAL SERVICES, INC. for a period of two (2) years from date of audit.
