Enclosed is the Pilgrim bi-weekly status report from the NRC Resident Office at Pilgrim.

The NRC Restart Staff is in place and was on-site monitoring licensee activities on an. extended shift basis during the licensee's Power Ascension Test Program.

In addition, a six member NRC Augmented Inspection Team'was onsite from April 13 through April 15, 1989, to review the sequence of events and the licensee's response to the r'elease of steam and water into the Reactor Core Isolation Cooling System room on April 12, 1989.

The Pilgrim bi-weekly status report is intended to provide NRC management and the public with an overview of plant activities and NRC restart inspection activities.

Subsequent inspection reports will address many of these topics in more detail.

ENCLOSURE Pilgrim Status Report for the Period April 8 - 21, 1989 1.0 Plant Status As of 8:00 a.m., on April 21, 1989, the reactor was in the cold shutdown mode with moderator temperature about 130 degrees F.

2.0 Power Ascension Test Program Status The licensee completed the Power Ascension Test Program up to the 25%

power plateau.

Prior to continuation of the power ascension program to the 50% power plateau, the licensee must obtain the NRC Region I Regional Administrator's concurrence.

The program includes NRC approval points ai 5%, 25%, 50 4 and 75*J of full power and both a licensee written report and NRC review af ter completion of testing at full power.

The NRC Restart-I Assessment Panel recommends to the Regional Administrator when the plant is ready to be released from each approval point.

3.0 Facilities Operations Summary A reported in the last Pilgrim Status Report (March 24 - April 7,1989),

' w plant was operating at approximately 24% power at the beginning of t:i i - report period.

At approximately 9:15 a.m., on April 12, 1989, the Reactor Core Isolation Cooling (RCIC) system injection valves opened unexpectedly during a test of the system logic.

The RCIC discharge check valve also apparently failed to properly seat, allowing backleakage of feedwater to the low pressure RCIC suction piping. The suction piping relief valve lifted, its discharge overflowed the RCIC room drains, and about two inches of water quickly accumulated on the room floor before operators shut the injection valves to terminate the event.

Five workers were slightly contaminated and were promptly decontaminated.

The RCIC system was isolated and declared inoperable pending further licensee evaluation.

By April 16, 1989, the licensee had completed those portions of their investigation which could be done with the reactor at normal operating pressure. The licensee declared an Unusual Event due to initiation of a plant shutdown at 1:35 p.m. on April 16, 1989, to continue investigation and repair of the possibly inoperable check valve in the RCIC systea. An Unusual Event is the lowest level of emergency classification and provides a low threshold for initial notification to offsite authorities.

The Unusual Event was terminated at 8:50 p.m.

There was minimal safety significance to this event declaration.

The plant remained in the cold shutdown mode throughout the remainder of the report period. Throughout the period, the licensee continued to con-duct routine surveillance along with preventivo and corrective maintenance.

4.0 Items of Special Interest NRC Augmented Inspection Team at Pilgr_im In response to the release of steam and water into the RCIC. compartment (i.e.,

room) on April 12, 1989, NRC Region I dispatched an Augmented Inspection Team (AIT) to Pilgrim station on April 13, 1989.

The AIT was led by Mr. Eugene Kelly, Chief, Technical Support Section, Division of Reactor Projects, and the team consisted of five technical sp'ecialists from NRC Region I, the Pilgrim resident staff and the Office of Nuclear Reactor Regulation. The inspection focused on the sequence of events, the causes, and the licensee's response to the event.

The Team held an exit meeting at the NRC Region I office with licensee management on April 19, 1989. A representative of the Commonwealth of Massachusetts was also in attendance at the exit meeting.

Results of the AIT will be documented in Inspection Report No. 50-293/

89-80.

5.0 Emergency Notification System (ENS) Reports Loss of Secondary Containment On April 13, 1989, a momentary loss of secondary containment occurred when the secondary containment doors for Fan Room #4 on the Reactor Building 74' elevation were opened simultaneously during a series of secondary containment door interlock inspections to ensure that two doors would not operate simultaneously. The interlock on these doors failed this test so that the two doors were opened simultaneously for a few seconds.

Both doors were immediately closed and steps taken to prevent recurrence until the door interlock is repaired.

The licensee made one ENS notification on April 12, 1989, and two on l

April 16, 1989, regarding the RCIC system event and subsequent plant shut-down described in Paragraph 3.0.

l 6.0 Pilgrim Restart Staff Activities The Pilgrim restart staff was maintained on extended shift coverage throughout the report period. Additional personnel from NRC Region I, and an NRC contractor were onsite during portions of the period to augmeni the resident inspection staff.

A NRC-developed inspection plan addressing the 5-25's power plateau and the l

primary functional areas of operations, maintenance, surveillance, and startup testing continued to be implemented.

The inspectors assessed operator performance, organizational interfaces, adherence to procedures and technical specification compliance.

In addition, following the reactor shutdown of April 16, 1989, the resident inspectors, region based inspectors and NRC contractor extensively followed the licensee's evalua-tions, analyses, repairs, modifications and post-work testing related to the RCIC event of April 12, 1989.

The Pilgrim Restart Staff continued to evaluate the performance of licen-see personnel and the plant to determine whether the licensee should be permitted to proceed to the next NRC approval point, a maximum of 50%

rated power.

The Restart Staff and the Pilgrim Restart Assessment Panel will review the April 12, 1989 RCIC system event before finalizing that determination.

Contact with the press and public continued throughout the period.

The NRC Restart Manager and the Region I Public Affairs Officer have been handling inquiries from the press and the public.
