Subject:

Vermont Yankee Supplemental Response to NRC Bulletin No. 88-10: Holded-Case Circuit Breakers Bulletin No. 88-10 [ Reference b)] requested that addressees take actions to provide reasonable assurance that molded-case Circuit Breakers (CBs), including CBs used with motor controllers, purchased for use in safety-related applica-l tions without verifiable traceability to the Circuit Breaker Manufacturer (CBM) perform their safety functions. By letter dated March 31, 1989 [ Reference f)],

Vermont Yankee submitted a written report outlining the results and actions taken'as a result of our investigation.

Pursuant to the commitments outlined in Reference f), Vermont Yankee hereby provides a status of all ongoing items as of June 30, 1989.

REPORTING REQUIREMENT 1 All holders of operating licenses are required to provide a written report by April 1, 1989 that:

Confirms that only molded-case CBs that meet the criteria of Item 7 of the action requested are being maintained as stored spares for future use in safety-related applications.

b.

Summarizes the total number, manufacturer, model number, and to the extent possible, the procurement chain of those CBs that could not be traced to the CBM in Items I and 4 of the actions requested. For installed CBs, also identify each system.in which they are/were installed.

If Item 4 of the actions requested has not been completed by April 1, 1989, due to the sche-dule for tests in Item 3 of the actions requested, this information should be updated within 30 days of the completion of Item 4 to address those additional CDs that could not be traced to the CBM.

c.

Confirms that Items 1, 2, 3, 4, 5, 6, and 7 of the actions requested have been completed or will be completed as requested.

Response

a.

As previously stated in our original response [ Reference f)], only molded-case CBs that meet the criteria of Item 7 of the Bulletin actions requested are being maintained as stored spares for future use in safety-related applications at Vermont Yankee. All molded-case CBs whose traceability to the CBM cannot be verified at this time, are being retained in segregated storage at our plant.

b.

As requested by the Bulletin, Vermont Yankee has identified all molded-case CBs purchased prior to August 1, 1988 tnat are being maintained as stored spares for safety-related (Class 1E) applications or commercial grade CBs that are being maintained as stored spares for future use in safety-related applications. Our review included CBs purchased directly from CBMs and l

from other sources.

As stated in our original response, Vermont Yankee was pursuing the tra-ceability of the 212 identified MCCB's.

The results of that effort are complete and summarized in Attachment 1 (MCCB's traceable to OEM) and (MCCB's not traceable to OEM).

It should be noted that an additional seven (7) breakers have been identified since our original response and three (3) traceable breakers originally identified were issued during the past refuel outage. This gives a total of 217 breakers, of j

which 121 (55%) are traceable.

c.

The following provides Vermont Yankee's actions or position with respect to each of the seven Actions Requested as identified in Bulletin 88-10.

Item 1 of the requested actions was completed in accordance with the Bulletin's schedular request (prior to March 1, 1989).

2.

Vermont Yankee has prepared, and has on file, an analysis justifying continued operation for the 9 installed MCCBs as identified in Items 4 and 7 until Items 1 through 5 of the actions requested have been completed.

3.

Item 3 of the requested actions is not applicable to Vermont Yankee since less than 80 percent of our MCCB's in stock were traceable to the OEM.

4.

As a result of our documentation search identifying less than 80% of the MCCBs being traceable, the following is our response:

a.

Vermort Yankee has identified all MCCB's purchased between August l

1, 1983, and August 1, 1938, installed in safety-related applica-

tions, b.

Vermont Yankee has verified traceability of all but 8 installed MCCB's as identified in Action 4.a.

c. identifies the number, manufacturer, model number, and system in which the non-traceable MCCB's are installed.

In addition, to the extent possible, the procurement chain for those non-traceable MCCB's is provided.

Vermont Yankee will replace the MCCB's identified in Action 4.c with MCCB's that meet the criteria of item 7 of the action requested or test them in accordance with the program described in Attachment 1 of Bulletin No. 88-10.

MCCB's that fail the test will be replaced with NCCB's that met the criteria of Item 7 of the actions requested or MCCB's that pass all tests, in accordance with the test program described in Attachment 1 of Bulletin No. 88-10.

These actions will be completed prior to the start-up from our next

'l scheduled refueling outage currently planned for September 1990.

6.

Vermont Yankee will comply with Item 6 of the requested actions of the Bulletin.

Information generated while performing the actions requested by the Bulletin will be maintained by Vermont Yankee for a period of five years after completion of all requested actions.

a.

Vermont Yankee has completed its review of MCCB's installed after August 1, 1988, and has identified one non-traceable installa-tion. This one breaker is listed on Attachment 3 and will be handled in accordance with the requirements of requested Action 5 of the Bulletin.

Vermont Yankee revised its purchasing process on February 13, 1989, to require that all electrical equipment be traceable back to the original equipment manufacturer.

In addition, all new purchase orders will require that any electrical equipment is newly manufactured and not refurbished.

REPORTING REQUIREMENT 2 "All holders of operating licenses are required to submit a report that summarizes available results of tests conducted in accordance with Item 3 and 5 of the actions requested within 30 days after startup from the first and second refueling outages beginning after March 1, 1989. For CBs that pass these tests, the only information required is the number, manufacturer, model number, and to the extent possible, the procurement chain of CBs tested (summary report format l

is acceptable).

For CBs that fail these test (s), these reports should indicate the test (s) and the values of test parameter (s) at which the failure (s) occurred, as well as the corresponding manufacturer, model number, and to the extent possible, the procurement chain."

Response

If any testing as specified in Bulletin No. 88-10 is performed, Vermont Yankee will submit a report summarizing any test conducted and the results in accordance with the criteria identified in the subject Bulletin.

REPORTING REQUIREMENTS 3 AND 4 Bulletin 88-10 Reporting Requirements 3 and 4 are directed to Holders of Construction Permits and are, therefore, not applicable to Vermont Yankee.

CONCLUSION i

1 In accordance with the requirements of NRC Bulletin No. 88-10, Vermont

Yankee has initiated a program to provide reasonable assurance that molded-case CBs purchased for use in safety-related applications without verifiable tra-ceability to the CBM can perform their safety function.

In addition, as requested in Reference c), we will retain any CBs which are identified as unac-ceptable for use in order to support further NRC review of this issue.

Further, Vermont Yankee will provide a second supplement to the response to Bulletin 88-10, identifying the status of any testing and/or replacement activi-ties. These actions will be completed in accordance with the requirements of the Bulletin with the supplemental response submitted no later than 30 days following start-up from our next scheduled refueling outage currently planned for September 1, 1990.
