SUBJECT:

Peach Bottom Atomi'c Power Station, Unit 2 Release from 35% Power Plateau On April 26, 1989,Section V.A of the. Shutdown Order issued March 31, 1987 suspending power. operation with respect to Peach Bottom Atomic Power' Station (PBAPS) was modified to permit operation of Unit 2 not to exceed 35 percent full power.

In accordance with the approved Restart Power Testing Program, Peach Bottom Atomic Power Station has accomplished the planned activities.at'the 35% power level and completed a comprehensive management review on June 23, 1989 of station equipment, personnel performance and management processes for the period April 26, 1989 through the 35% power assessment.

The PBAPS Restart Power Testing Program Management Oversight Team (MOT) met on June 1, 1989 after completion of the transient testing period with the plant at the 35% power level steady state plateau to review line management. assessments of operations,

' maintenance, services (chemistry, health physics, radwaste),

technical support and security.

The MOT met again on June 23, 1989 after three weeks of' operation at nominal 35% power.

The MOT reviewed plant operations, maintenance schedules and work progress, surveillance test completion and chemistry as well as s

open items from the previous meeting.

In addition, at both

]

meetings the MOT reviewed Nuclear Quality Assurance evaluations,

)

independent industry observer observations and MOT member i

observations which confirmed line management assessments.

As a result, the MOT concluded on June 23, 1989 that the physical plant

operate at nominal 70% power in accordance with the approved

. Restart Power Testing Program.

Specific conclusions for each area reviewed are. summarized below.

The-MOT discussed'several plant events in detail.

Based upon-a discussion.of the 1mproperly. installed wiring in the HPCI flow

~

controller circuitry, the MOT concluded that the root cause analysis determined this was an. isolated deficiency.

The investigation of the SCRAM resulting from.feedwater control

-problems indicated that.a control switch failed.

The switch has been replaced and sent.for outside analysis which is in progress.

The= MOT discussed the improper operation of a mainsteam line low

. pressure switch-which initiates a Group I isolation.

This switch.

will be replaced and monitored to ensure proper operation.

Also, reviewed by the MOTLwas the need to have a third feedpump available.

Plant management indicated a plan exists to have this pump available during the next assessment period.

The MOT concluded that the plant performed satisfactorily.

Operations strengths included good communications and cooperation between groups during major plant evolutions, prompt and thorough event and incident investigations, timely completion of procedural changes, preview of selected procedures prior to use, prompt and conservative actions taken in response to'off-normal conditions, and a quick adaptation to the operating environment.

An' additional strength was the " hot" license training for personnel holding " cold" licenses.

Routine daily operations-meeting effectiveness required improvement but was being acted upon by line management and is an area for continuing line management attention.

Another area identified for continuing line management attention is operator staffing levels with respect to simultaneous support of Unit 2 operations, Unit 2 thirteen (13) week rolling maintenance schedule, Unit 3 outage support, and operator progression training.

Maintenance strengths included a streamlined and more efficient organization, continuing acceptable plant material condition, and effective maintenance troubleshooting and repairs in response to problems.

The MOT cited a decreasing trend in maintenance' backlog and a significant improvement in the ability to meet the 13 week rolling maintenance schedule since June 1, 1989.

Areas. identified for continuing line management attention are the 13 week rolling maintenance schedule, improving personnel understanding of the Maintenance Request Form (MRF) priority system, and training shift I&C technicians to process MRFs.

Services (chemistry, health physics and radwaste) strengths included good and improving housekeeping, improving industrial safety practices, minimized spread of contamination and improved group communications.

i monitor copper. levels and conductivity in the feedwater which are presently at or.below specifications and decreasing.

identified for continuing line management attention.is improved 1

implementation of radiological practices by new workers.

Technical support strengths included good intergroup communications, improved procedural adherence,, knowledgeable and responsive personnel, and prompt identification of problems and completion of correcFive actions.

An area identified for continuing line marigi. ment attention' included refinement to the reporting process ter completed surveillance tests.

Security strengths included improvement in personnel performance, improvement in personnel training, reduction in the number of recordable events, improvement in contingency drill participation, reduction in compensatory posting, improvement in reliability of detection equipment and reduction in incidents involving uncontrolled badges.

Areas for continuing management attention by multiple line organizations included improving feedback to shift' managers by support groups, improving radio communications protocol by non-operating personnel and improving timely completion of scheduled routine work within.the 13 week rolling maintenance schedule.

ANI, NRC and Commonwealth of Pennsylvania, State of Maryland and co-owner representatives who attended one or both of the MOT meetings were asked to provide their comments.

No adverse comments were forthcoming.

The MOT reviewed outstanding action items from the June 1, 1989 meeting and determined that closure was satisfactory or that continuation of these items along with those established at the June 23, 1989 meeting would not adversely impact the safe operation of Unit 2.

In consideration of the above, we are requesting that the Regional Administrator, Region I modify the March 31, 1987 order to permit operation of Unit 2 at nominal 70% full power in accordance with the approved Restart Power Testing Program.
