" PRECURSOR" OPERATIONAL EVENTS WHICH OCCURRED DURING TH PERIOD NOVEMBER 1,1987 THRU MARCH,1988 Introduction This Technical Report lists and summarizes the precursor events that have been culled from operational LER data for the period November 1,1987 thru March,1988 by the Accident Sequence Precursor (ASP) Program. Note that because of delays of submitting and processing LERs, this list of precursors is not exhaustive and additional precursors for the interval will likely be included in future reports. Furthermore, about 15% of LERs are revised hence future information received from the utilities could effect precursor selections and quantifications.

The precursors listed in the attached table are events that are important elements in postulated nuclear plant accident sequences.

They can involve nuclear plant safety system failures or degradations, i.e., be " Unavailability" events found by surveillance testing. Precursor events can also involve an accident or initiator requiring plant safey system response. These two event types are denoted as "U" and "I" respectively in the accompanying table.

Results There were 12 precursor type LERs during this period plus two potential precursors and four LERs involving containment or containment systems. The four containment type LERs do not directly impact core damage likelihood but they can impact radiological release likelihood and consequences of postulated accidents. These four containment events are net evaluated for significance due to lack of appropriate models. Three of the events during this time period involved support systems. The events considered as support system I

events were the Instrument Air failure at Davis Besse, the Emergency Service Water failures et Harris 1, and the circulating water pump problems at N.

18 Note that six of the precursor type events occured or were discovered at 0%

power. Two of the six zero power events involved containment. The remaining four zero power events are considered significant to the extent they could have occured while the reactor was at power.

Observation There are not enough precursor events for this short interval to draw conclusions regarding precursor trends or regarding the factors which could influence the types and significance of precursor occurences. An observation is that the PWRs may be somewhat more vulnerable to support system problems than BWRs. Also,.the containment or containment systems events seem to be a significant fraction of the total. The ASP program has only recently begun to document and track containment and containment system failures or degradations in anticipation of performing operational data evaluations in the context of the Safety Goal. Therefore it isn't known whether these containment event occurrences are typical or not.

The precursors in the attached table seem to indicate that the "Frontline" k

high pressure systems (ECI, AFW, etc.) are most frequently involved with l

significant events. This may indicate that low pressure systems deserve somewhat less attention in plant testing, maintenance, and technical specification requirements. A further analysis of precursor events is planned for a future report which will consider events occurring over a longer time period to try to determine some more basic implications of the precursor event occurences.
