i The following information is provided to finalize the DLC response to Bulletin 88-10.

Reporting Requirement 1.b:

l Summarize the total number, manufacturer, model number, and to l

the extent

possible, the procurement chain of those'CBs.that could not be traced to the CBM in items 1 and 4 of the actions requested.

For iratalled CBs, also identify each system in which they are/were installed.

If item 4 of the actions requested has not been completed by April 1, 1989, due to the schec'le for tests in item 3 of the actions requested, this information should be updated within 30 days of the completion of item 4

to address those additional CBs that could not be traced to the CBM.

Supplemental Response:

Attachment 1 to the April 2, 1989 response identified a stored circuit

breaker, stock number 834640, as having unconfirmed q

traceability.

This breaker has since been found to have

)

convincing physical-evidence of tampering.

It was purchaued from Stearns-Roger for use in fuel handling equipment.

Vendor records show that WESCO obtained it from Aaker Electrical.

Supply of

Passaic, N.J. and in turn sold it to Stearns-Roger

{

(the primary vendor).

The Westinghouse Model Number is FA3015 (obsolete).

listed installed breakers which were confirmed untraceable or not currently proven traceable.

The current status of these items is:

1 All 10 breakers.were replaced prior to the ITE/JL3 I

BVPS-2 startup from refueling.

Heinemann/CD3-Z42 These breakers were custom-made for DLC since-they were not a normal Heinemann catalog item.

Therefore, they could only have been obtained'from the manufacturer.

All others -

Traceability has been established.

This concludes our response to this bulletin.
