TECHNICAL PEVIEW RE RT AE0D/T710 ATING, VENTILATING AND AIR CONDITIONING qTFM D _ Mg This memo and the enclosed report are in response to Ed Jordan's request for the generic dimensions of the problems associated with HVAC systems. The request was prompted by an SCRS letter to the EDO dated October 15, 1987, related to HVAC systems. Thi letter is attached to this memo for your information.

To identify the generic aspects of the problems with HVAC systems, we searched the SCSS data base for events involving HVAC systems and other safety systems A quick review of the 183 events thus obtained is documented in the attached report. The generic aspects of the problem can be categorized on the basis of three root causes. Two roet causes, design deficiency and personnel error, have already been the subject of identified generic issues and AE0D case studies. USI A-44, Station Blackout; GI 50, BWR Vessel Level Instrumentation; AE0D/C604, Effects of High Ambient Temperature on Electronic Components in I&C Systems; AE00/C605, Losses of Electrical Inverters, AEOD/C701, Instrument Air System Problems; and the ongoing AE0D study of Service Water System Problems are examples of such issues and studies. The third root cause--component and equipment failures -- is recommended to be the focus of a more detailed study by AEOD. Such a study would also identify the other root causes and could be used to verify the adequacy of ongoing actions in correcting them. No immediate actions are needed at this point regarding the identified problems.

I have the abrtracts of the 183 events which can be used as the starting data base for the future study.
