l II. Haukeaan Memorial Airport Expansion Plans:

The airport master. record (FAA 5010-1 Report), and airport map (FAA 5090 i

Report) has been updated and submitted by the Haukegan Port Administrator.

The airports. environmental impact assessment report, submitted in 1978 has been approved.

III. Annual Occupational Exposure Data Personal Radiation Exposure Data per Reg. Guide 1.16 is reported under separate cover by the Health Physics b partment.

IV. Challenges to Primary System PORV and Safety Valves:

There were two occurrences of Pressurizer Power Operated Relief Valve (PORV) actuation in 1988. One actuation occurred on March 29, 1988.

During periodic testing of Safeguards Actuation, an inadvertent phase A Conta'wnt Isolation occurred, resulting in isolation of instrument air to Containment. During a subsequen' ramp down in power, both PORV's lifted and reseated due to the loss of air to Pressurizer Spray Valves.

This event is documented in License Event Report (LER) 304/88-003-00.

A second incident of PORV actuation occurred on Zion Unit 2 on December 11, 1988 while performing modification testing under Technical Staff Special Procedure (TSSP) 148 @ after an inadvertent Safety Injection.

All components actuated as required.

Primary system pressure was properly maintained by the PORV's during the SI.

Since the unit was in cold shutdown there were no adverse thermal effects on the plant piping.

This event was reported as LER 304/88-012-00.

The health and safety of the public were not affected by the items described in this report.

Modifications A.

Refueling 1 The unit two refueling outage that started in October 1988 ended in December 1988.

Fuel was loaded in 1988.

Unit one refueling outage started in February 1988 and ended in May 1988, fuel was loaded in 1988.

B.

Modifications Four reportable modifications were completed and verified to operate properly.

The modifications are:

1.

dqdification Description - M22-0-86-17 Provide raised center desk in Control Room.

This modification raised the center desk area approximately 1 foot and gives the Station Control Room Engineer clear view of both control boards and provides for more effective routing of personnel traffic in the Control Room. Spaces for computer terminals, files and table space were incorporated into the design.

This Modification satisfies DCRDR HED 0010, 0011, 0216 and 0298.

Safety Evaluation Summary This Modification is non-safety related, non-EO, but cabinets and raised floor supports are mounted seismically to the existing floor.

This Modification does not interact with any system that is safety related and does not involve any Tech Spec or FSAR issues.

2.

Modification Description - M22-0-87-11 Manual switches in gas analyzer cabinet.

This Modification permanently lifts the leads to solenoid valves on the sample lines to Unit 1 and 2 VCT's because the sample valve can be opened inadvertently by hitting adjacent buttons on the panel.

Safety Evaluation Summary The lifted leads make the system safer by preventing inadvertent release through sample valves of radioactive waste gas. VCT gas space sampling is not addressed in the Tech Spec's.

Modification Description - M22-1-84-04 Modify Rx trip breakers per NRC Reg. and HOG recommendations.

This Modification does not alter the existing reactor protection, but adds the auto shunt trip capability.

The reactor trip breaker shunt coils were replaced with qualified coils.

4 Safety Evaluation Summary This change adds extra protection features.

If more breaker cycles are required for testing, then it is possible that the breakers will have to be replaced more frequently.

However, this should not impact breaker performance.

This change is being done compatibly with previous existing features.

4 Modification Description - M22-1-86-23 Eliminate points on by-pass and permissive.

This Modification satisfied NRC concerns related to DCRDR HED 172, 348, & 369; l

requiring simplification and reduction of Unneeded Annunciation on the bypass and permissive Annunciator Panel on the Unit 1 Control Board.

The remaining Annunciation on this Panel will be changed to the Dark Board Concept.

l Safetv Evaluation Summary This Modification only involves Annunciator alarms, which does not affect FSAR accidents nor Tech Spec basis margin of Safety.

II.

Tests i

The only testing conducted was done per Tech Spec requirements which involved Performance Testing and Surveillance Testing as required by Zion Station Technical Specifications.

III. Experiments There were no experiments conducted that involved a change in the Technical Specifications or unreviewed safety questions.
