Admission or Denial of'the Alleged Violation TVA admits'that example 2 of the violation occurred in that actions were not taken in a timely manner.

Reason for the Denial if Admitted TVA focused their attention on the four temporarily promoted Shift Operator Supervisors (SOSs) and failed to do further investigation early on the other four'50Ss identified in the employee concern report from the new Employee Concern Program. After realizing that the form 7860 report on student and operator training activities did not exist when these four SOSs were examined, TVA relied on the recollection of the personnel involved rather than following through to search for some documentation of successful accreditation examination completion.

Corrective Steps Which Have Been Taken and Results Achieved A records search was initiated and documentation (accreditation exam cover sheets) were located for the four other SOSs.
Since September 1979, form 7860 is issued upon completion of all upgrade Accreditation Examinations. Copies are distributed to Personnel Records, Nuclear Training Records, a Plant Management / Labor representative, and to the Operator Training _ Department.

Corrective Steps Which Will Be Taken to Avoid Further Violations TVA will continue to take a proactive approach to factoring concerns from the New Employee Concern Program into the line organizations.
In turn, the line organization will be careful to ensure that additional concerns are addressed along with the initial issue.
