Regarding Action Plan Results Report I.b.2 In accordance with the. Board's Memorandum; Proposed Memorandum and Order of April 14, 1986, the Applicants submit the answers of the Comanche Peak Response Team ("CPRT") to the 14 questions posed by the Board, with respect to the Results Report published by the CPRT in respect of CPRT Action Plan I.b.2, Flexible Conduit to Cable Separation.

The CPRT inspection of the Unit 1 control boards and vertical-ventilation panels utilized inspection reports which are attached to CPRT~ Quality Instruction QI-OO4, "CPRT ACTION PLAN I.b.1 - Flexible Conduit to Flexible Conduit Separation; CPRT ACTION PLAN I.b.2 -
Flexible Conduit.to Cable Separation."
All of the questions below dealing.with checklists are answered with respect to this inspection procedure.
The CPRT also utilized checklists in the compilation of multi-train panels.
One of the actions requested.by the NRC was a reinspection ~of all panels that contain redundant conduits.
In order to determine the scope of this activity, it was necessary to identify all panels that contain more than one train.
To do this, the CPRT Electrical Review Team reviewed all Unit 1 electrical drawings to determine which panels contained cables from more than one train.
This list was then used by the CPRT to examine the multi-t,

s, train panels to determine where Servicair flex _had been used as a barrier.
The process that was used to compile the list of multi-train panels is described in the ISAP I.b.1 Working File,-in particular, Documents I.b.1-6B.009 and I,b.1-6B.011.
Since these checklists were only used to determine the scope of the multi-train panels and were not used in inspection activities to determine the acceptance or rejection of safety-related attributes, Questions 2 through 9 are not relevant to this review checklist.
Question:
1.
Describe the problem areas addressed in the report.
Prior to undertaking to address those areas through sampling, what did Applicants do to define the problem areas further?
How did'it believe the problems arose?
What did it discover about the QA/QC documentation for those areas?
How extensive did it believe the problems were?
Response
The original issue raised by the NRC-TRT involved exposed cables in the control room panels that were in contact with Servicair flex containing redundant cables.
This configuration was in violation of existing Project requirements, which specified a one-inch separation.
In addition, the NRC questioned the 1
i use of Servicair flexible metallic conduit as a barrier.
If Servicair flex could not:be shown to be a
_ barrier, a six-inch separation was to be: maintained,

which is the separation distance for two exposed cables.
Furthermore, the NRC stated that "TUEC shall also reinspect all remaining panels in the control room and other areas of the plant containing separate cables and cables within flexible conduit The first issue that was addressed was the adequacy of Servicair flex as a barrier.
This evaluation was required to determine-the appropriate separation _between an exposed cable and a cable in Servicair flex.
This task was performed in conjunction with the testing and analysis performed in Action Plan I.b.l.
The Results Report for I.b.2 describes the steps that were taken to demonstrate the adequacy of l
Servicair flex as a barrier.
Though it was assumed at the start of this action plan that Servicair flex was used only in the control boards and vertical ventilation panels, it became apparent that Servicair flex had been used in other electrical panels.
Rather than use sampling techniques, the Electrical Review Team examined all l

multi-train panels, i.e.,. panels that contain more than one electrical train.
These are the only panels that would have used Servicair flex as a barrier.
-The CPRT third-party examination of multi-train panels identified all cases where the final CPRT separation criteria for exposed cables and cables in Servicair flex had been violated.
In addition, although the NRC only addressed the case of an exposed cable and Servicair flex, the CPRT third-party found that other separation criteria were not being met in the control room panels.
The CPRT third-party did not determine a priori that QA/QC activities or any other activities were responsible for the identified-deviations.
This investigation of root causes was performed during.the implementation of Action Plan I.b.4.
The generic implication evaluation of the root causes to determine the extent to which other electrical panels could similarly be affected was also performed under Action Plan I.b.4.
Based on this evaluation, it was determined that a reinspection of all electrical panels i.

As noted above, the inspection report used by the CPRT third-party inspectors during their inspection of the control boards and vertical ventilation panels was a part of Quality Instruction QI-004.
All revisions of
.this procedure are included in File I.b.1-3 of the ISAP I.b.1 Working File.
In addition, all background information used to develop'the criteria in the procedure are included in this file.

The Unit 1 control boards and vertical ventilation panels were inspected shortly before the CPRT third-party inspection.
During the Project inspection, the procedure used was QI-QP-11.3-40, Revisions 19, 20 and 21, " Post Construction Inspection of Electrical Equipment and Raceways."
In addition,.$

in-process inspections had been previously performed in

accordance with QI-QP-11.3-28, " Class 1E Cable Terminations."
As noted in the Results Report, preliminary, conservative inspection guidelines were used during the CPRT inspection.
This was done so as to provide an indication of the extent of potential modifications required for compliance with the final CPRT separation criteria.
In addition, the information obtained from this inspection was used in the testing and circuit evaluation performed to demonstrate the adequacy of Servicair flex as-a barrier.
The separation distances specified in the CPRT inspection guidelines were greater than or equal to the distances specified in the criteria in QI-QP-11.3-40 and QI-QP-ll.3-28.

Explain the extent to which the checklists contain fewer attributes than are required for conformance to codes to which Applicants are committed to conform.

With one exception, the CPRT Inspection Report included a check of all physical separation attributes - -

1 required by'IEEE 384-1974, "IEEE Trial-Use Standard Criteria'for Separation of Class-1E Equipment and Circuits,"'and~ Regulatory Guide 1.75, Revision 1,
" Physical Independence of Electric Systems."
The one

exception is that the separation of cables, each enclosed ~in a vendor wireway, was not verified.
Since-the Project criteria at that time allowed two redundant-cables in metallic. conduits to touch, the metallic wireways used in the control boards and vertical ventilation panels are allowed to be in contact with
- one another.
Therefore, no inspection of separation between vendor wirebays was required.

The following revisions were made to the CPRT inspection procedure QI-004:
Initial issue 1/18/85 Revision 1 2/18/85 Revision 2 2/26/86 Revision 3 3/6/85 Revision 4 3/12/85 Revision 4 of QI-004 was used for all control boards and vertical ventilation panels except control boards CP1-ECPRCB-01 thru -04, which were inspected to Revision 2 and control boards cpl-ECPRCB-05 and -07, which were inspected to Revision 3.
Since the first revision used in an actual inspection was Revision 2, changes made prior to Revinion 2 do not affect the inspections performed.
In addition, changes made subsequent to Revision 2 of the procedure do not affect the panels inspected to revisions earlier than Revision 4.

Revision 3 changed the inspection method for missing barriers.
Rather than perform a paper trace to determine if documentation exists for the J

~ installation / reinstallation of missing barriers, the inspector was to determine whether or not the barrier is required for separation purposes.
In addition, changes were made in the way in which the Inspection Reports were to be completed-for control boards CP1-ECPROB-10 and -11 and the vertical ventilation panels (CPX-ECPRCV-01 and -03).

Revision 4 provided additional clarification on the inspection criteria for control boards CP1-ECPRCB-10 and -11 and the vertical ventilation panels (CPX-ECPRCV-01 and -03).
Since control boards CP1-ECPRCB-10 and -11 and the vertical _ ventilation panels (CPX-ECPRCV-01 and -03) were inspected to Revision 4 of QI-004, the changes F4.d not affect the inspections performed to Revisions 2 and 3 of QI-004.

Classroom and field training were conducted for Revision 0 of QI-004.-
A 50-minute classroom session and a one-hour field course were held for inspectors.
Training was-performed for Revision 2 of QI-004, the first revision actually used in the inspection of the panels.
Revisions 3 and 4 contained minor changes that were addressed in formal discussions with the inspectors.

Provide any information in Applicants' possession concerning the accuracy of use of the checklists (or the inter-observer reliability in using the checklists).
Were there any time periods in which checklists were used with questionable
. training or QA/QC supervision?
If applicable, are problems of inter-observer reliability addressed statistically?
Response
There is no information available concerning the accuracy of the use of the checklists or inter-observer reliability in using the checklists.
The subject inspections were conducted primarily in the first quarter of 1985; there were no time periods in which the checklists were used with questionable training or Mw
l QA/QC supervision.
All-inspectors were certified Level II or above in accordance with the requirements of ANSI N45.2.6-1978 and Regulatory Guide 1.58, Revision 1.

Summarize all audits or supervisory reviews (including reviews by employees or consultants) of training or of use of the checklists.
Provide the factual basis for believing that the audit and review activity was adequate and that each concern of the audit and review teams has been resolved in a way that is consistent with the validity of conclusions.
Responses No ERC Corporate Audits or Supervisory Reviews were conducted which were directly related to the use of the checklists or training applicable to this action plan.
However, ERC Corporate Audits 85-01 and 85-03 and QA Surveillances 185-01 and III85-09 verified that ERC inspectors were qualified and certified in accordance with ANSI N45.2.6-1978 and Regulatory Guide 1.58, Revision 1.
In addition, applicable ERC inspectors received classroom instruction and on-the-job training in the requirements of QI-004, which is the instruction which governed the reinspection effort.

Response
Neither Mr. Bizzak (the Issue Coordinator) nor Mr. Mallanda (the Review Team Leader) could recall any substantive changes during the generation, review, and editing of the report resulting from management action.
Accordingly, our answer to this question is:

Set forth any unexpected difficulties that were encountered in completing the work of each task force and that would be helpful to the Board in understanding the process by which conclusions were reached.
How were each of there unexpected difficulties resolved?
Response
Several unexpected difficulties were encountered during the implementation of this action plan that resulted in delaying the completion of this action plan.
These difficulties can be categorized as 4

problems arising from the sequencing of tasks, technical problems, andrinterface problems.
~
The problems arising due to the sequencing of
' tasks were due in large part to the initial schedule developed'for the resolution of.the issue.
To accommodate this schedule. several related activities were performed simultaneously rather than sequentially, which resulted in extra effort.
The optimum approach would have been to determine the scope of the issue, i.e.,
where had Servicair flex been used; perform circuit evaluations of the panels where Servicair flex existed; conduct tests for those c.ases in which the circuit evaluation could not be used to demonstrate acceptability of flex as a barrier; issue criteria; and perform reinspections to the new criteria.
During the formulation of the action plan, the assumption was made that Servicair flex had been used only in the control boards and vertical ventilation panels.
The action plan tasks were then initially formulated to only address these panels.
Though the assumption that flex is only used in the control boards and vertical ventilation panels is true for the majority of the Servicair flex used, flex was later found in other panels.
This necessitated a new task to identify all multi-train panels and a subsequent examination of these panels to determine where Servicair flex had been used.
Circuit evaluations, testing, and inspections of the control boards were performed during the same time period.
Since criteria resulting from the circuit evaluation and testing were not finalized, the inspections utilized preliminary guidelines that were confusing.
Subsequent work had to be performed to determine if the findings against the preliminary guidelines were also violations of the final CPRT criteria.
The testing that was performed was based on a typical installation, but did not include a full range of current levels or the worst configuration.
Due to this, subsequent parametric computer analyses were required to demonstrate the adequacy of Servicair flex.
The technical problems involved the testing and analyses performed for the Servicair flex.
As noted above, the initial test configuration selected was not for the worst case.
The subsequent time-history computer analyses took much longer to complete than :
anticipated.
In addition, it was found after the analyses were completed that an out-of-date version of the computer code had been used, which resulted in some of the analyses being redone.
The last area of difficulty involved the interface between the CPRT and the Project.
The interface was strained at times which resulted in an inefficient use of resources.
Disagreements on the need to perform certain activities, such as the identification of multi-train panels to determine where Servicair flex might have been used, resulted in the CPRT third-party performing these activities.
Rather than coordinate activities with the CPRT third-party, the Project placed its emphasis on resolving the issue along a parallel path using Wyle test data, which was not provided to the third-party.
The lack of emphasis on the completion of the CPRT action plan shown by the Project resulted in the CPRT third-party spending excessive time to close out the issues, especially where the Project's input was required.
However, none of the above items had an effect on the activities or results of this ISAP.

Response
There are three potential areas in the Results Report that could be confusing to someone not familiar with the' subject of the Action Plan:
- 1) the chronology of events;-2) the basis for the different types of
-inspections performed; and 3) the technical details of the analyses performed to demonstrate the adequacy of Servicair flex as a barrier.
Due to the simultaneous activities performed for several different, interrelated tasks, i.e.,
inspection of separation in the control boards and vertical ventilation panels, evaluation of circuits, and testing of Servicair flex, the chronology of events is difficult to follow.
This possible confusion was noted l
during the review of Revision O of the Results Report, and a chronology of events was added in Revision 1 to clarify the time phasing of the various activities.
During the implementation of the I,b.2 Action Plan, several different physical reviews of electrical panels were performed; some of these reviews utilized different criteria.
In addition, previous or future inspections of the panels were referenced in the Results Report.

QI-004 RVision 4
3.0 RESPONSIBILITIES 3.1 The third party inspectors (ERC) shall be responsible for conducting inspections and for completion of inspection reports. The Electrical / Instrumentation Review Team Leader is responsible for the review and approval of the inspection reports.
4.0 INSTRUCTION 4.1 The Quality Inspector shall inspect the specific Main Control boards and vertical panels (Ref. 1.1) to verify that:
ACTIVITY 1 -Internal lighting and 120 VAC convenience receptacle wiring not routed in rigid conduit. DIT, or Unistrut F-1000 channel is separated at least 6 inches from safety related wiring and components (Ref. 1.1, para. 3 and Ref. 1.5).
ACTIVITY 2 -Flexible conduit attached to control switch modules does not exceed 3 feet in length without additional support (Ref.
1.1, para. 2 and Ref. 1.3).
ACTIVITY 3 -Verify that wiring is separated in accordance with, Ref 1.1, para. 4, 5. Fig.1 and Table 1 of Ref.1.3.
ACTIVITY 4 -Wiring is mounted and supported such that minimum separation will be maintained (Ref. 1.1, para. 2 and Ref. 1.3).
ACTIVITY 5 -The correct type barrier asterial is installed (Ref. 1.1, Note).
ACTIVITY 6 -Verify that " required" barriers are installed.
Note:
" Required" means either required by a design document, or indications that a barrier was installed but has been tenoved.
The inspector will state the location of missing barriers and whether or not a separation violation has been created by the barrier's absence.
4.2 For Sections C and D only of the inspected control boards, the Quality taspector shall identify all exposed non-Tefsel wiring (Ref. 1.1, para.
1 and Ref. 1.2).
4.3 DOCISIENTAT!uN 4.3.1 The Quality Inspectors will be furnished with the applicable drawings and documents required to perform activities in paragraphs 4.1 and 4.2.
4.3.2 The Quality Inspectors shall document the results and inspections for para. 4.1 on the IR (Attachments 2 and 3 as applicable). For para. 4.2, the Quality Inspectors shall identify exposed non-Tefsel wiring on Attachment 4 (Supplement Sheet) and attach the form to the appropriate IR.

Jones (CPP-17,914) 2.0 PURPOSE AND SCOPE 2.1 The purpose of this Quality Instruction is to define the inspection activities, methods, and documentation requirements necessary to assure that adequate physical separation of wiring has been maintained, and that required barriers have not been removed without authorization.
This effort is in direct support of CPRT Action Items I.b.1 and I.b.2.
2.2 The scope of this procedure is the cabling and wiring within selected control panels and cabinets as identified in the above references. The scope of inspections is any interaction between safety related wiring and components, excluding vendor wireway to vendor wireway (per Ref.
1.5).
NOTE:
The scope of this Quality Instruction may be expanded at the direction of the Electrical / Instrumentation Review Team Leader.
3.0 RESPONSIBILITIES l

3.1 The third party inspectors (ERC) shall be responsible for conducting inspections and for completion of inspection reports. The Electrical / Instrumentation Review Team Leader is responsible for the review and approval of the inspection reports.
4.0 INSTRUCTION 4.1 The Quality Inspector shall inspect the specific Main Control boards and vertical panels (Ref. 1.1) to verify that:
ACTIVITY 1 -Internal lighting and 120 VAC convenience receptacle wiring not routed in rigid conduit, ENT, or Unistrut P-1000 channel is separated at least 6 inches from safety related wiring and components (Ref. 1.1, para. 3 and Ref. 1.5).
ACTIVITY 2 -Flexible conduit attached to control switch modules does not exceed 3 feet in length without additional support (Ref.
1.1, para. 2 and Ref. 1.3).
ACTIVITY 3 -Verify that wiring is separated in accordance with, Ref 1.1, para. 4, 5, Fig. I and Table 1 of Ref. 1.3.
-ACTIVITY 4 -Wiring is mounted and supported such that minimum sepgration will be maintained (Ref. 1.1, para. 2 and Ref. 1.3).
ACTIVITY 5 -The correct type barrier material is installed (Ref. 1.1, Note).
ACTIVITY 6 -Verify that " required" barriers are installed.
Note:
" Required" means either required by a design document, or indications that a barrier was installed but has been removed.
The inspector will state the location of nissing barriers and whether or not a separation violation has been created by the barrier's absence.
4.2 For Sections C and D only of the inspected control boards, the Quality Inspector shall identify all exposed non-Tefsel wiring (Ref. 1.1, para.
I and Raf. 1.2).
4.3 DOCUMENTATION 4.3.1 The Quality Inspectors will be furnished with the applicable drawings and documents required to perform activities in paragraphs l
4.1 and 4.2.
4.3.2 The Quality Inspectors shall document the results and inspections for para. 4.1 on the IR (Attachments 2 and 3, as applicable). For i
para. 4.2, the Quality Inspectors shall identify exposed non-Tefsel wiring on Attachment 4 (Supplement Sheet) and attach the form to the appropriate IR.

4.3 DOCUMENTATION (Cont'd) 4.3.3 The inspection documents (Attachments 2, 3 and 4) shall be complaced in accordance with their respective completion instructions, as applicable. The inspector completing the report shall indicate that all applicable inspections have been completed and documented correctly by entering signature, date and badge number. The IR's shall be reviewed for legibility, accuracy, and completeness and approved by the ERC Level III. The completed IR shall be submittaa to the Electrical / Instrumentation Review Teaa
' Leader for review and approval.
'4.3.4 All section a and b criteria blocks and inspection results blocks for panels ICB-10, ICB-11, XCV-01 and XCV-03 will be NA'd per Ref.

This effort is in direct support of CPRT Action Items I.b.1 and I.b.2.
202 The scope of this procedure is the cabling and wiring within selected control 2-panels and cabinets as identified in the above references. The scope of inspections i
is any interaction between safety related wiring and components and other safety related or non-safety related wiring and components, excluding vendor wiring to vendor wireway (per Ref.1.5).
NOTE: The scope of this Quality Instruction may be expanded at the direction of the Electrical / Instrumentation Review Team Leader.
3.0 RESPONSIBILITIES l
3.1 The third party inspectors (ERC) shall be responsible for conducting inspections and for completion of inspection reports.
The Electrical / Instrumentation Review Team Leader is responsible for the review and aporoval of the inspection reports.

4.0 INSTRUCTION 4.1 The Quality Inspector shall inspect the specific Main Control boards and vertical panels (Ref.1.1) to verify that:
ACTIVITY 1 - Internal lighting and 120 VAC convenience receptacle wiring not routed in rigid conduit, EMT, or Unistrut P-1000 channel is separated at least 6 inches from safety related wiring and components (Ref. 1.1, t
para. 3 and Ref. 1.5).
ACTIVITY 2 - Flexible conduit attached to control switch modules does not exceed 3 feet in length without additional support (Ref.1.1, para. 2 and Ref. 1. 3 ).
ACTIVITY 3 - Separation is maintained in accordance with Ref.1.1, para. 4 and 5 and Figure 1, Ref.1.3, Table 1 (Attachment 1).
ACTIVITY 4 - Wiring is mounted and supported such that minimum separation will be maintained (Ref.1.1, para. 2 and Ref.1.3).
ACTIVITY 5 - The correct type barrier material is installed (Ref.1.1, Note)..
ACTIVITY 6 - If it is obvious that separation barriers have been removed (i.e.
brackets installed but no barriers, etc.), or if design drawings require barriers that are not installed, verify that these barriers have not been removed without authorization (Ref.1.4).
ACTIVITY 7 - If barriers have been removed or are not installed, verify that documents exist requiring installation / reinstallation of the barriers (Ref. 1.4).
4.2 For Sections C and D only of the inspected control boards, the Quality Inspector shall identify all exposed non-Tefzel wiring (Ref.1.1, para,1 and Ref.1.2).

4.3 DOCUMENTATION 4.3.1 The Quality Inspectors will be furnished with the applicable drawings and documents required to perfonn activities in paragraphs 4.1 and 4.2.
l 4.3.2 The Quality Inspectors shall document the results of inspections for para. 4.1 on l
the IR (Attachments 2 and 3, as applicable). For para. 4.2, the Quality Inspectors shall identify exposed non-Tefzel wiring on Attachment 4 (Supplement Sheet) and attach the form to the appropriate IR.
4.3.3 The inspection documents (Attachments 2, 3 and.4) shall be completed in accordance with their respective completion instructions, as applicable. The inspector com-plating the report shall indicate that all applicable inspections have been completed and documented correctly by entering signature, date and badge number. The IR's shall be reviewed for legibility, accuracy, and completeness and approved by the ERC Level III.
The completed IR shall be submitted to the Electrical / Instrumentation Review Team Leader for review and approval.

This is in direct support of the CPRT Action Items I.b.1 and I.b.2.

NOTE: If internal lighting and 120 VAC convenience receptacle wiring is not routed in rigid conduit. EMT, or Unistrut P-1000 channels, N/A Activities 3 and 4 and complete Activity 5.
ACTIVITY #4 - Internal lighting and 120 VAC convenience receptacle wiring routed in rigid conduit, EMT, or Unistrut P-1000 channel is separated at least 1 inch from safety related components (Ref. 1.1, para 3).
ACTIVITY #5 - Internal lighting and 120 VAC convenience receptacle wiring not routed in rigid conduit, EMT, or Unistrut P-1000 channel is separated at least 6 inches from safety related wiring and components (Ref. 1.1, para 3).

.4.3.1 The Quality Inspectors will be furnished with the applicable drawings and documents required to perform activities in paragraphs 4.1 and 4.2.
4.3.2 The Quality Inspectors shall document the results of inspections for para. 4.1 on the I.R. (Attachment 2). For para 4.2 the Quality Inspectors shall identify exposed non-Tefzel wiring on Attachment 4 supplement sheet and attach the fann to the appropriate I.R.
4.3.3 The inspection documents (Attachments 2 and 4) shall be completed in accor-dance with Attachments 3 and 5, as applicable. The I.R. 's shall be re-viewed for legibility, accuracy, and completeness and approved by the ERC Level III. 2.1 The purpose of this Quality Instruction is to define the method by which
to ensure that required separation is maintained within specified Main Control room panels and that no separation barriers have been removed with-out authorization. This is in direct support of the CPRT Action Items I.b.1 and I.b.2.
2.2 The scope of this Quality Instruction is to verify by inspection, that required seoarction is maintained and that no separation barriers have been remor A without authorization.
3.0 RESP 0aSIBILITIES 3.1 The third party inspectors (ERC) shall be responsible for conducting inspec-tions and for completion of inspection reports. The Electrical /Instru-mentation Review Team Leader is responsible for the review and approval of the inspection reports.
l 4.0 INSTRUCTION l
i 4.1 The Quality Inspector shall inspect the specific Main Control boards and vertical panels (Ref.1.1) to verify that:
Activity #1 - Wiring is mounted and supported such that minimum separation will be maintained (Ref.1.1, para. 2).
Activity #2 - Flexible conduit attached to control switch modules does not exceed 3 feet in length without additional support (Ref.1.1, para.2).
Activity #3 - Internal lighting and 120 VAC convenience receptacle wiring is routed in rigid conduit, EMT, or Unistrut P-1000 channel (Ref.1.1, para 3).

NOTE:
If internal lighting and 120 VAC convenience receptacle wiring is routed in rigid conduit. EMT, or Unistrut P-1000 channel, N/A Activity 5 and complete Activities 3 and 4.
Activity #6 - Separation is maintained in accordance with Ref.1.1, para 4 and 5. Table 1 and Figure 1 (Attachment 1)..
NOTE:
Inspector shall note the specific drawing and detail used for verift-cation in the appropriate block of the I.R.
Activity #7 - The correct type barrier material is installed. (Ref.1.1, Note).
Activity #8 - If it is obvious that separation barriers have been removed (i.e. brackets installed but no barriers, etc.), or if design drawings require barriers that are not installed, verify that these barriers have not been removed without authorization.
Activity #9 - If barriers have been removed or are not installed, verify that documents exist requiring installation /reinsta11ation of the barrier (s).
4.2 For Sections C and D only of the inspected control boards, the Quality Inspector shall identify.all non-Tefzel wiring (Ref.1.1, para 1).
l 4.3 DOCUMENTATION i
4.3.1 The Quality Inspectors will be furnished with the applicable drawings and documents required to perform activities in paragraphs 4.1 and 4.2.
4.3.2 The Quality Inspectors shall document the results of inspections for para 4.1 on the I.R. (Attachment 2).
For para 4.2, the Quality Inspectors 1
j shall identify non-Tefzel wiring on Attachment 4 supplement sheet and l
attach the form to the appropriate I.R.
4.3.3 The inspection documents (Attachments 2 and 4) shall be completed in accor-dance with Attachments 3 and 5, as applicable. The I.R.'s shall be re-F viewed for legibility, accuracy, and completeness and approved by the ERC l
Level III. The completed I.R. shall be submitted to the Electrical /Instru-l mentation Review Team Leader for review and approval.
