Gentlemen:
This refers to Certificate of Compliance No. 5754 for the Modei No. RMG.172 packaging which has an expiration date'of September 30, 1989..
To ensure continued-use of this package, an. application for renewal (6 copies) of the existing certificate should be submitted at-least 30 days: prior;to the expiration date of the certificate. The section on operating procedures,-
i acceptance tests, and' maintenance programs should be re m wed to assure they are complete and current.' If revisions are necessary the revised' sections-i should be submitted with the application for renewal.' Note that the certificate l
of compliance may be conditioned to specifically cite the operating procedures, l
acceptance testing and maintenance programs.
j Unless an exemption is provided by 10 CFR Part 170.11, an application ~ fee of
$150.00, as required by 10 CFR Part 170.31, should be included with the request j
for renewal. This notice of expiration should not be construed that such j
notice will be provided in the future. Also, please let us know if you do not intend to request renewal of the certificate.
