Louisiana Power & Light Company _

Docket: 50-382 Waterford Steam Electric Station, Unit 3 Operating License: NPF-38 During an NRC inspection conducted April- 16 through May 31,.1989, two d violations of NRC requirements were identified. The first violation involved a failure to provide maintenance instructions appropriate to the circumstances.

The second violation involved a failure to follow procedures.. In accordance j and Procedure for NRC Enforcement i with Actions," the "10General CFR Part Statement of Policy 2 Appendix C (1988), the vi6f ations are listed below: I A. Inadequate Maintenance Procedure Technical Specification 6.8.1.a requires, in part, that written procedures ll shall be established, implemented, and maintained as recommended inc Appendix A of Regulatory Guide 1.33, Revision 2, February 1978.

Appendix A to Regulatory Guide 1.33 recommends maintenance on safety-related equipment to be performed in accordance with written procedures appropriate to the circumstances. 1 Contrary to the above, on May 11 and 12,1989, the instmetions in Work Authorization 01037167 were not appropriate and resulted in incorrect washer stacking on the torque switch terminals and improper power supply cable splices during motor actuator reassembly for Valve MS-416.

B. Failure to Follow Radiological Protection Control Procedures .!

Technical Specification 6.11.1 requires, in part, that- approved procedures j for personnel radiation protection shbil be adhered to for all operations  ;

involving personnel radiation exposure. Paragraph 4.8.6 of approved l radiation protection Procedure HP-1-110, Revision 8 " Radiation Work <

Permits," requires observing and abiding by radiation work permits and posted radiological signs. Standing Radiation Work Permit (RWP) 89000002 l requires Health Physics permission prior to entering radiologically i restricted or high radiation areas. In addition, RWP 89000002 requires full anti-contamination protective clothing when climbing into contaminated areas.

Contrary to the above, on May 26, 1989, an auxiliary operator entered the Low Pressure Safety Injection Pump area, which had radiological pos*,.ings stating " Radiological Restricted Area," "High Radiation Area," and 'No Entry Without Health Physics Permission," without obtaining Health Physics permission. In addition, several operations personnel were observed climbing in areas posted as contaminated without wearing required full protective clothing.

Louisiana Power & Light Company Pursuant to the provisions of 10 CFR 2.201, Louisiana Power & Light Company is hereby required to submit to this office, within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the reason for the violations if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
