SUBJECT:
3M CONTAMINATED SITE SURVEY FORMS Approximately 4200 3M static eliminator customer sites have been surveyed for polonium-210 contamination to date.
It is our understanding from 3M that this constitutes all sites wnere leaking, returned static eliminators have been identified. Region III is not aware of any facilities which are known to have had contaminated devices for which no site survey was performed.
Three hundred forty-three of the sites were identified to be contaminated, to varyirg degrees. Attached are survey report forms for e&ch of those contaminated sites, compiled alphabetically by State.
This information should have been conveyed by 3M to appropriate regional offices and Agreement States at tho time of the surveys so this compilation will serve to confirm those contaminated sites.
Region III recommends that these survey forms be distributed to the appropriate Region or Agreement State.
If additional information is needed about a particular site's survey and decontamination, please contact Mr. James Lynch, Region III 3M Project Manager, at FTS 388-5669 or (312) 790-5669. Mr. Lynch will request, in writing, further information from 3M in regard to the subject general licensee site. At this time, we are pursuing one such request from Region I.
