III. Annual Occupational Exposure Data Personal Radiation Exposure Data per Reg. Guide 1.16 is reported under separate cover by the Health Physics Department.
IV. Challenges to Primary System PORV and Safety Valves:
One incident of PORV actuation occurred on Zion Unit 2 on December 11, 1988 while performing modification testing under Technical Staff Special Procedure (TSSP) 148-88 after an inadvertent Safety Injection. This event was reported as LER 88-012-00.
All components actuated as required. Primary system pressure was properly maintained by the PORV's during the SI. Since the unit was in cold shutdown there was no adverse thermal effects on the plant piping.
The' health and safety of the public were not affected by the items described in this report.

Dear Mr. Sorrentino,
The attached information was sent to me by the F.A.A. I believe
.this will meet your needs.
As you will note, the 5090 report is the actual map of the airport; the 5010-1 report is the Airport Master Record. Another change is that Mr. Denison is no' longer Airport Manager, and.at this time Mr. Al Huisman is the Acting Airport Manager.
Sincerely, Mary Walker i i
